Home My WebLink About20033352.tiff RESOLUTION RE: APPROVE CHANGE ORDER LETTERS#1 AND#2 FOR TUBERCULOSIS PROGRAM AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with Change Order Letters#1 and #2 for the Tuberculosis Program between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment, commencing July 1, 2003, and ending June 30, 2004,with further terms and conditions being as stated in said change order letters, and WHEREAS,after review,the Board deems it advisable to approve said change order letters, copies of which are attached hereto and incorporated herein by reference. NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County,Colorado,that Change Order Letters#1 and#2 for the Tuberculosis Program between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment be, and hereby are, approved. BE IT FURTHER RESOLVED by the Board that the Chair be,and hereby is,authorized to sign said change order letters. Wallace, MD, MPH, Director Department of Public Health and Environment •COLORADO DATE: December 8,2003 --)/V\0AAN,5 /tAX-ree0ta, SUBJECT: Change Order Letters #1 and #2 fort e Tuberculosis Program Enclosed for Board review and approval are change order letters number one and number two for the Tuberculosis Program. If approved,these letters will become part of the master contract between the Weld County Department of Public Health and Environment (WCDPHE) and the Colorado Department of Public Health and Environment(CDPHE). WCDPHE provides active TB treatment, including directly observed therapy; ensures contacts to all newly identified infectious TB cases are identified and receive appropriate evaluation; collects specimens for testing on all persons suspected of having TB; provides for and interprets chest x-rays; and conducts TB skin test screening programs. Health Department staff also follows up with active patients to ensure they receive appropriate information and education and they adhere to the completion of their TB therapy. These two change order letters will provide additional funding for the TB program in the amount of$6,797 through letter number one and $4,357 through letter number two. The letters will bring the total amount of funding for this program from the CDPHE to a total of$67,443 for the time period July 1, 2003 through June 30, 2004.
