HomeMy WebLinkAbout20030748.tiff RESOLUTION RE: APPROVE LETTER OF AGREEMENT FOR PREGNANCY RISK ASSESSMENT MONITORING SYSTEM DATA ANALYSIS AND AUTHORIZE CHAIR TO SIGN - PRIME SOLUTIONS WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Letter of Agreement for Pregnancy Risk Assessment Monitoring System Data Analysis between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and Prime Solutions, with terms and conditions being as stated in said agreement, and WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Letter of Agreement for Pregnancy Risk Assessment Monitoring System Data Analysis between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and Prime Solutions be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said agreement. Jerke ty tt rney Glenn Va Date of signature: 2003-0748 HL0030 0 i February 1, 2003 Gaye Akin, Health Promotion Supervisor Weld County Department of Health & Environment 1555 N. 17th Avenue Greeley, CO 80631 Dear Ms. Akin: This letter of agreement sets forth the mutual understanding with respect to the consulting services (The 2003 Weld County PRAMS Study) to be performed by Cindy Ann ICronauge, MPH CHES, dba as Prime Solutions for Weld County Department of Public Health (WCDPHE) and Environment. Prime Solutions has outlined the following scope of work plan to conduct the 2003 Weld County PRAMS Study, to which the WCDPHE has agreed, the following services will be performed: 1. Meet with PRAMS workgroup as needed during January 2003 to June 2003 to develop study questions and focus of data analyses, and prepare final report summaries. 2. Meet and correspond as needed with CDPHE Health Statistics Section representatives to obtain data file and accompanying technical information. 3. Analyze the Weld County PRAMS data file'. 4. Prepare a detailed data summary report of findings of the Weld County PRAMS data file intended for use by Weld County agencies serving women and children, policy makers, elected officials, program planners and grant writers. 5. Prepare an executive summary report including recommendations about improving services to women and children intended for Weld County business owners, CEOs, and benefits managers. 6. Meet and correspond with WCDPHE personnel including the Health Promotion Supervisor, Health Planners, and Weld County PRAMS Study project assistant as needed to complete the study in a timely manner. 'WCDPHE agrees to allow Prime Solutions access to department computers that have SAS and SUDAAN software for the analysis. A preliminary timeline for the project is contingent upon receipt of the data file from CDPHE by February 15, 2003 and is outlined below: 2003 January February March April May June WORKGROUP focus study questions x x review initial data output x determine additional x analyses review additional analyses x review/edit draft reports x x release/publicize reports x STAFF obtain data x run initial analyses x run additional analyses x prepare draft reports x x prepare final reports x x In order to conduct the 2003 Weld County PRAMS Study, WCDPHE agrees to disclose certain information concerning Weld County, its organization, policies, and procedures. Prime Solutions considers all such information about Weld County and the recommendations, reports or other work produced during the performance of the services, to be confidential information. Only authorized representatives of Prime Solutions will be given access to this information unless explicit permission is obtained in advance from appropriate Weld County representatives. Any subcontractors, assistants, or interns of Prime Solutions that assist in the performance of the 2003 Weld County PRAMS Study will sign a confidentiality agreement with respect to this project. Ownership of data,reports or other works produced by Prime Solutions for WCDPHE shall be the property of WCDPHE with Prime Solutions being credited with authorship. Should Prime Solutions and WCDPHE decide that it would be advantageous to publish the reports or summary or any portion of the reports, the determination of this may be made jointly by both parties, provided that Prime Solutions may publish the results as a case study with no identification of WCDPHE if no other agreement is reached. In consideration of the services performed, WCDPHE will pay Prime Solutions, a total amount of$9,500.00 over the term of the agreement. Weld County, Colorado will be invoiced in three payments, $3,3000.00 on February 15, 2003, and $3,1 00.00 on April 1, 2003 and $3,100.00 at the expected completion date of June 15, 2003. The final payment will not be made until the project has been satisfactorily completed. If the project has not been timely and satisfactorily completed, WCDPHE, after giving Prime Solutions a 10-day written notice, may terminate this agreement with Prime Solutions and may retain the services of another party to complete the project. Prime Solutions will promptly and within 5 days of date of termination, provide to WCDPHE any and all work performed on the project pursuant to this agreement by Prime Solutions or under its supervision, contract or direct, up to the date of termination. This agreement is approved and accepted by all parties and is executed by the signatures of authorized representatives of all parties. PRAMS is funded by the Colorado Department of Public Health and Environment, Health Statistic Division, to collect and analyze data on the acceptability and availability of health care for pregnant women and new born infants in Weld County and Colorado.
