RE: APPROVE TASK ORDER CHANGE ORDER LETTER #3 FOR TUBERCULOSIS
PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with Task Order Change Order Letter#3 for the
Tuberculosis Program between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment,
commencing July 1, 2003, and ending June 30, 2004,with further terms and conditions being as
stated in said task order change order letter, and
WHEREAS,after review,the Board deems it advisable to approve said task order change
order letter, a copy of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,that Task Order Change Order Letter#3 forthe Tuberculosis Program between
the County of Weld,State of Colorado,by and through the Board of County Commissioners of Weld
County, on behalf of the Weld County Department of Public Health and Environment, and the
Colorado Department of Public Health and Environment be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is,authorized to
sign said change order letters.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 31st day of March, A.D., 2004, nunc pro tunc July 1, 2003.

Task Order Change Order Letter Number 3, Contract Routing Number 04 FHA 00298
State Fiscal Year 2004-2004, Tuberculosis Prosram
This Task Order Change Order Letter is issued pursuant to paragraph F_3.of the Master Contract identified
as contract routing number 00 FAA 00008 and paragraph C.4. of the Task Order identified as contract
routing number 04 FHA 00029 and contract encumbrance number PO FHA EPI0400029. This Task
Order Change Order Letter is between the COLORADO DEPARTMENT OF PUBLIC HEALTH AND
ENVIRONMENT and BOARD OF COUNTY COMMISSIONERS OF WELD COUNTY (a political
subdivision of the state of Colorado). The Task Order has been amended by Task Order Change Order
Letter 1, contract routing number 04 FHA 0000198,and by Task Order Change Order Letter L contract
routing number 04 FHA 00201. The Task Order,as amended, if applicable, is referred to as the"Original
Task Order". This Task Order Change Order Letter is for the current term of July 01,2003,through
June 30,2004. The maximum amount payable by the State for the work to be performed by the Contractor
during this current term is increased by TEN THOUSAND DOLLARS,($10,000.00)for an amended
total financial obligation of the State of SEVENTY SEVEN THOUSAND FOUR HUNDRED FORTY
THREE DOLLARS,($77,443.00). The revised specifications to the revised Budget are incorporated
herein by this reference,made a part hereof, and attached hereto as"Attachment A". The first sentence in
paragraph C_1.of the Original Task Order is modified accordingly. All other terms and conditions of the
Original Task Order are reaffirmed. This change to the Task Order shall be effective upon approval by the
State controller,or designee, or on April 19,2004,whichever is later.
Please sign, date, and return all 3 originals of this Task Order Change Order Letter by March 31,2004,to
the attention of: Tanya Schrimpscher,Colorado Department of Public Health and Environment,4300
Cherry Creek Drive South,Mail Code DCEED—A3,Denver,Colorado 80246. One original of this
Task Order Change Order Letter will be returned to you when fully approved.

valid until the State Controller,or such assistant as he may delegate,has signed it.The contractor is

SUBJECT: Tuberculosis Task Order Change Order
Letter#3
Enclosed for Board review and approval is Change Order Letter#1 to the task order for the
Tuberculosis Program. If approved,this change order will become part of the master contract
between the Colorado Department of Public Health and Environment (CDPHE) and Weld
County Department of Public Health and Environment (WCDPHE).
This change order letter will provide an additional $10,000 from the State of Colorado for the
Tuberculosis Program. The funding will be used for physician consultation, increasing TB
outreach worker hours and nurse visits, implementing contact investigation, providing case
isolation, and ensuring active patients receive appropriate education and adhere to their TB
therapy.
With the approval of this change order letter,the total amount of funding for this program will be
$77,443 for the time period of July 1, 2003, through June 30, 2004. I recommend your approval.
