Dear Sir or Madam:
This office represents Christine Weckwerth of Greeley, Colorado with regard to the notice given hereby to
the County of Weld, Colorado, and its employees, of our client's claims arising out of the slip and fall
accident which occurred on August 10, 2004 on the sidewalk in front of the building at 915 Tenth Street.
Our office is furnishing you the following information with regard to our client's claim.
IDENTITY OF CLAIMANTS
The claimant is Christine Weckwerth who has an address of 1601 57th Avenue,Greeley,CO 80634 and has a

BASIS OF CLAIM
Our client's claim against the County of Weld, Colorado and its employees is based upon the hospital,
medical and rehabilitative expenses and pain and suffering and loss of enjoyment of life for a total of
$10,000. The County and its employees failed to properly monitor and maintain the services at the entrance
to 915 Tenth Street, Greeley, Colorado, thereby allowing a sizable break in the sidewalk to occur. Such
action on the part of the County of Weld was negligent and was a direct cause of the damages suffered by
our client.
NAMES OF PUBLIC EMPLOYEES
The claimant is unaware of all of the names and addresses of the public employees who engaged in the
conduct or admissions as described above.
NATURE AND EXTENT OF INJURY OR DAMAGES
The damages to our client are estimated to be in the amount of$10,000.
