RESOLUTION
RE: APPROVAL OF CONTRACT BETWEEN THE WELD COUNTY DEPARTMENT OF
SOCIAL SERVICES AND THE WELD COUNTY DENTAL SOCIETY.
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, a dental contract for 1980 between the Weld County
Department of Social Services and the Weld County Dental Society
has been presented to the Board of County Commissioners of Weld
County, Colorado, and
WHEREAS, the Board of County Commissioners has studied said
contract and deems it advisable and in the best interests of Weld
County to approve the same.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Com-
missioners of Weld County, Colorado that the dental contract for
1980 between the Weld County Department of Social Services and the
Weld County Dental Society be, and hereby is, approved.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 23th day of
January, A.D. , 1980.

1980 DENTAL CONTRACT

This contract executed at Greeley, Colorado this 1' day of December, 1979, by and
between the Board of County Commissioners of Weld County, Colorado, hereinafter called
the County, and the Weld County Dental Society, hereinafter called the Society:
WITNESSETH:
That in consideration of the mutual promises and obligations and other considerations
herein expressed, it is mutually agreed until December 31, 1930:
1. The Weld County Dental Society shall furnish dental services as outlined in the
fee schedule for county indigents. Approved billings shall be submitted bi-monthly.
Payment is to be made to the Weld County Dental Society. The budgeted annual cost
for this service is $15,000.00. If this amount should become depleted, no pro-
vision is made for continued services.
2. The Society will, so far as it lies within the power of its members, see that proper
dental care and service are rendered to all indigents in Weld County.
3. The Society will appoint an administrative committee to assist in checking billings
for services rendered.
4. The Society will appoint its own committee, empowered to make all arrangements to
properly administer and fairly divide the funds among participating members render-
ing service, and conduct the business of the Society under this contract. The de-
cision of this committee shall be final and binding to all doctors who participate ,
in this contract. It is expressly understood, however, that any member of the
Weld County Dental Society may be present at any meeting of such committee and
have full opportunity to examine the books to see that the funds are divided fairly ,
and without prejudice among all participating doctors who have performed services.
The members of this committee act only as agent for the Society and in no case shall
they be held liable.
5. Prompt billing for completed services is imperative. No authorization for payment
shall be made for billings submitted more than 120 days after date of service.
Any doctor/participant suspected of irregular billings or falsifications of his
records may be disqualified from participating in the contract by the Administrative
Committee.
6. Maximum adult dental services coverage shall be $80.00 per year. Denture repairs
not covered under programs such as the Old Age Pension Dental Program, will be
authorized to a limit of $80.00 per year.
Weld County indigents who receive dental services or denture repairs under this
contract shall be liable for 10% of the authorized service amount, to a maximum of
$8.00.
THIS CONTRACT OTHERWISE EXCLUDES DENTURES OR OTHER EXTENSIVE SPECIALIZED SERVICES.
7. It is understood by all parties to this contract that TITLE XIX of the Social
Security Act (MEDICAID) provides dental services foi all children of eligible AFDC
public assistance recipients up to the ace of 21. Also, that there may be Dental
and/or Denture services available to Old Age Pension recipients under a separate,
State of Colorado program. Billings for any services rendered children covered by
this MEDICAID or State program shall be made directly to the designated fiscal agent
by the provider.
8. Anv unexpended funds at the end of the year 1930 shall revert to Weld County, Coloradc

the County in its proper name and by its proper authorized officers, attested
by its Clerk and Seal, and said Society by its proper name and proper officers
thereunto duly authorized by it, the date first shown mentioned.

Please find attached the 1980 dental contract between the Weld County Department
of Social Services and.the.-Weld.County'Dmntal Society. We are requesting your
review and submittal to the Coaanty_Coaissbners for approval. This contract is
similar to those approved by the Board in 'previous years, except for certain cost
increases due to inflatiaa. The County Board approved. $15,000. for this Department
to pay for this service in our 1980 budget.
