The Board of County Commissioners of Weld County, Colorado, met in regular session as the Board of
Social Services in full conformity with the laws of the State of Colorado at the regular place of meeting in
the Weld County Centennial Center, Greeley, Colorado, December 22, 2004, at the hour of 9:00 a.m.
ROLL CALL: The meeting was called to order by the Chair and on roll call the following members were
present, constituting a quorum of the members thereof:
Commissioner Robert D. Masden, Chair
Commissioner William H. Jerke, Pro-Tem
Commissioner M. J. Geile
Commissioner David E. Long
Commissioner Glenn Vaad
Also present:
County Attorney, Bruce T. Barker
Acting Clerk to the Board, Debra S. Miller
Controller, Barbara Eurich
MINUTES: Commissioner Vaad moved to approve the minutes of the Board of Social Services meeting
of December 20,2004,as printed. Commissioner Long seconded the motion,and it carried unanimously.
WARRANTS: Barbara Eurich,Controller, presented the warrants for approval by the Board in the amount
of $89,885.16. Commissioner Jerke moved to approve the warrants as presented by Ms. Eurich.
Commissioner Geile seconded the motion, which carried unanimously.
NEW BUSINESS:
CONSIDER COLORADO WORKS PROGRAM AGREEMENT FOR SERVICES AND AUTHORIZE CHAIR
TO SIGN-THE COMMUNITY FOUNDATION SERVING GREELEYAND WELD COUNTY: Judy Griego,
Director, Department of Social Services, stated the agreement is for calendar year 2005. She explained
Leanna's Closet will provide emergency clothing to Colorado Works Program female clients who are
seeking employment or maintaining employment, and the County will reimburse Leanna's Closet at a rate
of$20.00 per client. Commissioner Vaad moved to approve said agreement. Seconded by Commissioner
Geile, the motion carried unanimously.

CONSIDER CHILD AND ADULT PROTECTION AGREEMENT FOR LEGAL SERVICES AND AUTHORIZE
CHAIR TO SIGN -WELD COUNTY ATTORNEY: Ms. Griego stated the Weld County Attorney will assist
the Department of Social Services in providing legal services necessary to comply with state laws and
regulations concerning care for children and adults, including any necessary court action. She stated the
source of funding is Child Welfare and County Administration funding. Commissioner Long moved to
approve said agreement. Seconded by Commissioner Geile, the motion carried unanimously.
CONSIDER AGREEMENT FOR SERVICES AND AUTHORIZE CHAIR TO SIGN - WELD FOOD BANK:
Ms. Griego explained the Weld Food Bank will provide food baskets for Colorado Works or Temporary
Assistance for Needy Families'eligible clients. She stated reimbursement to Weld Food Bank is$20.00
per food basket,with a maximum reimbursement of$10,000. Commissioner Vaad moved to approve said
agreement. Seconded by Commissioner Long, the motion carried unanimously.
CONSIDER GENERAL ASSISTANCE AGREEMENT FOR SERVICES AND AUTHORIZE CHAIR TO SIGN-
WELD COUNTYASSOCIATION OF OPTOMETRISTS AND OPHTHALMOLOGISTS: Ms.Griego stated
the Weld County Association of Optometrists and Ophthalmologists agrees to provide vision services at
a maximum total reimbursement rate of$22,000 per year. Commissioner Jerke moved to approve said
agreement. Seconded by Commissioner Long, the motion carried unanimously.
CONSIDER CHILD SUPPORT ENFORCEMENT COOPERATIVE REIMBURSEMENTAGREEMENTAND
AUTHORIZE CHAIR TO SIGN-WELD COUNTYATTORNEY: Ms.Griego stated the Weld County Attorney
will provide legal services necessary to establish paternity and support obligations and other child
enforcement matters. Commissioner Geile moved to approve said agreement. Seconded by
Commissioner Vaad, the motion carried unanimously.
Let the minutes reflect that the above and foregoing actions were attested to and respectfully submitted by
the Acting Clerk to the Board.
