RE: APPROVE TASK ORDER CHANGE ORDER LETTER 1 FOR THE IMMUNIZATION
PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Task Order Change Order Letter 1 for the
Immunization Program between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment,
commencing January 1, 2004, and ending December 31, 2004,with further terms and conditions
being as stated in said task order change order letter 1, and
WHEREAS, after review,the Board deems it advisable to approve said task orderchange
order letter 1, a copy of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Task Order Change Order Letter 1 for the Immunization Program
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said task order.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 20th day of October, A.D., 2004.

SUBJECT: Change Order Letter for the
Immunization Program
Enclosed for Board review and approval is a task order change order letter between the Colorado
Department of Public Health and Environment(CDPHE) and Weld County for the Immunization
Program.
This change order letter will provide additional funding in the amount of$36,341 and will be
used to provide community education regarding the rules of the Colorado Board of Health
pertaining to the immunization of students attending school and rules pertaining to the infant
immunization program. WCDPHE will also conduct reminder/recall activities, will collaborate
with the State's Supplemental Nutrition Program (WIC), will serve as a resource for the Weld
County Immunization Coalition, and will provide walk-in immunization clinics each month for
persons of all ages.
With this increase, the total amount of funding for the Immunization Program will be $83,728
for the time period of January 1, 2004 through December 31, 2004.
I recommend your approval of this change order letter.

State Fiscal Year 2004-2005, Immunization Program
This Task Order Change Order Letter is issued pursuant to paragraph F. of the Master Contract identified by
contract routing number 00 FAA 00008 and paragraph C. 4. of the Task Order identified by contract routing
number 01 FAA 01014 and contract encumbrance number PO FAA IMM0101014. This Task Order Change Order
Letter is between the COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT and
BOARD OF COUNTY COMMISSIONERS OF WELD COUNTY. The Task Order has been amended by Task
Order Option to Renew Letter 01,contract routing number 02 FAA 00827,and Task Order Option to Renew Letter
02,contract routing number 03 FAA 00677, and Task Order Option to Renew Letter 03, contract routing number 04
FHA 00178. The Task Order,as amended, if applicable, is referred to as the"Original Task Order". This Task
Order Change Order Letter is for the current term of January 1,2004,through December 31,2004. The
maximum amount payable by the State for the work to be performed by the Contractor during this current term is
increased by Thirty-Six Thousand Three Hundred Forty-One Dollars, ($36,341.00)for an amended total
financial obligation of the State for this current term of Eighty-Three Thousand Seven Hundred Twenty-Eight
DOLLARS,($83,728.00). The revised Budget for this current term is incorporated herein by reference, made a
part hereof,and attached hereto as"Attachment 1". The first sentence in paragraph Cl..of the Original Task
Order is modified accordingly. All other terms and conditions of the Original Task Order are reaffirmed. This Task
Order Change Order Letter is effective upon approval by the State Controller, or designee, or on November 1,2004,
whichever is later.
Please sign,date,and return all five(5)originals of this Task Order Change Order Letter by October 15,2004,to
the attention of: Erikka Apolinar,Colorado Department of Public Health and Environment,Mail Code IMM-
A4,4300 Cherry Creek Drive South,Denver,Colorado 80246-1530. One original of this Task Order Change
Order Letter will be returned to you when fully approved.
