RE: APPROVE AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
AND AUTHORIZE CHAIR TO SIGN - WESTERN PATHOLOGY CONSULTANTS,
INC./MAYFIELD
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to a Child Protection
Agreement for Services between the County of Weld,State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and
Western Pathology Consultants, Inc./Mayfield,commencing January 30,2004,and ending May 31,
2004, with further terms and conditions being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Amendment to a Child Protection
Agreement for Services between the County of Weld,State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and
Western Pathology Consultants, Inc./Mayfield be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded,adopted by
the following vote on the 14th day of June, A.D., 2004, nunc pro tunc January 30, 2004.

RE: Amendment to Child Protection Agreement for Services Between the
Weld County Department of Social Services and Western Pathology
Consultants, Inc. (WPCI)/Mayfield
Enclosed for Board approval is an Amendment to a Child Protection Agreement for Services
between the Weld County Department of Social Services(Department) and WPCl/Mayfield. The
Amendment was discussed and reviewed at the Board's Work Session of May 3, 2004.
The major provisions of the Amendment are as follows:
1. Our original provider for alcohol and drug testing was Mayfield. Mayfield was acquired
by WPCl/Mayfield. The amendment provides for the transfer of the Agreement to
WPCl/Mayfield effective January 30, 2004.
2. The Amendment provides for minimal changes to the drug and alcohol drug testing fees.
3. The original Agreement ending date of February 28, 2004, was extended to May 31,
2004.
All other terms of the Agreement remain the same including the funding level of$30,000.

AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF SOCIAL SERVICES AND
WPCl/MAYFIELD
This Amendment,made and entered into the day of May 2004,by and between the Board of County
Commissioners, on behalf of the Weld County Department of Social services,hereinafter referred to as"Social Services,"
and Western Pathology Consultants Inc. (WPCI)/Mayfield,hereinafter referred to as the"WPCI/Mayfield"
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement(03-CPS-29)in order to acknowledge the
acquisition of Mayfield by Western Pathology Consultants,Inc. and to extend the term of the original Agreement;and
WHEREAS, Social Services has budgeted, appropriated, and otherwise made available;and
WHEREAS,required approval, clearance, and coordination have been accomplished from and with appropriate
agencies.
NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree as follow:
1. The parties of the Agreement,as amended,will be Social Services a WPCI/MSielicl, Mayfield and
its holdings were acquired by WPCI/Mayfield as of Mar 4. ,)V+-a'W`--'
2. The term of the Agreement shall be extended from its original expiration date of February 28,2004,to
May 31,2004.
3. The designated representative of WPCI/Mayfield shall be changed from Laura Mayfield to Misty West,
Chief Operations Manager.
4. Exhibit C,Collection Fee, sill be revised as follows:
Instant 5 panel Drug Screen.Tests for: THC, COC, AMP,OPIATES,PCP-$12.00
Instant 5 panel sent to lab for confirmation using a chemically unrelated method-$25.00
Standard Laboratory Test.Tests for: ALC,AMP/METH,BENZ,COC, OPI,PCP,THC$12.00
Club Drug Panel.Tests for: ECSTASY, GHB,ICETHAMINE, OXYCODONE-$48.50
Hair Test-Tests for: THC,COC,OPIATES,PCP,AMP-$90.00
Breathalyzer Screening-$2.00
Breathalyzer Confirmation if screening test was POS-$2.00
WPCI-Court Testimony-$75.00 per hour
This Amendment, together with the original Child Protection Agreement for Services, constitutes the entire
understanding among the parties with respect to the subject matter hereof, and may not be changed or modified except as
stated in the Child Protection Agreement for Services.
IN WITNESS WHEREOF,the parties hereto have duly executed the Agreement as of the day,month, and year
first above written.
