RE: APPROVE CONTRACT FOR WAIT TRAINING AND AUTHORIZE CHAIR TO SIGN -
ABSTINENCE AND RELATIONSHIP TRAINING CENTER
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a contract for WAIT Training between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Weld County Department of Public Health and Environment, and the
Abstinence and Relationship Training Center, commencing June 21, 2004, and ending June 22,
2004, with further terms and conditions being as stated in said contract, and
WHEREAS,after review,the Board deems it advisable to approve said contract, a copy of
which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the contract for WAIT Training between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Department of Public Health and Environment,and the Abstinence and Relationship
Training Center be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said contract.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 26th day of May, A.D., 2004.

SUBJECT: Contract for Wait Training
Enclosed for Board review and approval is a contract between the Weld County Department of
Public Health and Environment and the Abstinence and Relationship Training Center. This
contract in the amount of$11,575 is for trainer fees and curricula for WAIT training scheduled
on June 21st and 22nd for 30 youth pastors and teachers from Weld County.
The WAIT training curricula will be purchased with funds from the Title V Grant received from
the Colorado Department of Public Health and Environment in January 2004 for abstinence and
relationship education.

The ARTC will provide a trainer for participants of WAIT Training, to be
held on June 21 and 22, 2004
Travel Expenses/Misc. :

*Participants attending the entire training are certified and licensed to teach WAIT Training and use the

A 25% deposit of guarantee is due upon signature of the contract in the amount of$2893.75. Deposit is non-
refundable if sponsor cancels contract. Should the event be cancelled due to weather, the trainer will retain 1/2
trainer fee. Balance due upon completion of contract. The balance may be adjusted at the completion of the
seminar to reflect the actual number of participants. Checks made payable to: Abstinence & Relationship
Training Center, Inc. Sponsor acknowledges that he/she has and approved all of the conditions of this
