A SERVICE ENHANCEMENT REQUEST NO. 2005-01 FOR SUPPLEMENTAL SERVICES A C S` Name of Client: WELD COUNTY Date of Request: December 9, 2004 Related Agreement: Master OnSite Services Agreement between Weld County and ACS Government Systems, Inc. (ACS)dated November 1993 All terms defined in the Agreement and its amendments shall have the same meaning ascribed to them therein when used in this Service Enhancement Request. Pursuant to Section 2.2 of the Agreement, Client hereby requests that ACS perform the Supplemental Services described below. The parties acknowledge that any Supplemental Services to be performed pursuant to this Service Enhancement Request shall be performed under the terms and conditions of the Agreement. In addition, unless otherwise set forth below, any payments to be made by Client to ACS under this Service Enhancement Request shall be made pursuant to the terms and conditions of the Agreement. The amounts to be paid by Client to ACS under this Service Enhancement Request shall be in addition to any other amounts which Client is obligated to pay to ACS under the Agreement. Description of Supplemental Services: ACS will provide one additional FTE with the skills necessary to assist in supporting Weld County's e-government environment. The current County web site consists of 2172 pages, 963 images, with approximately 40 major sections. Additionally, we are running a number of ARCIMS map projects on multiple servers and a self-contained Intranet Site for county employees. We are supporting Restaurant Inspections,Treasurer Tax Lookup, Warrant lookup, Registered Sex Offenders, Weekly crime update, Webstaff, Employee Survey, Voter Lookup, Share your photos, Subscribe to Newsletter, File On-line complaints, Health Survey, Volunteer Registration, Citizen Corp Registration, Code Violation On-line, Pre-Trial Supervision Application, Jail Exit Survey, and On-line County Phonebook. We currently have requests for stand-alone websites for the Abstinence Education program, Headstart, and the County Fair. New requests arrive each month. An additional FTE would allow us to complete requests in a timely fashion, monitor usage more frequently, and provide backup for our existing position. The new resource will be entitled to standard ACS benefits and will deliver standard 40 hour work weeks. This Service Enhancement Request will be effective January 1, 2005, with charges prorated based on the actual start date of the individual filling the position. The SER will be coterminous with the master agreement dated November 1993. ACS will submit an invoice to the Client on a monthly basis beginning in January 2005 in twelve equal monthly payments based on the annual payment schedule listed below.
