RE: APPROVE APPLICATION FOR COMMUNITY FOOD AND NUTRITION PROGRAM AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Application for Community Food and
Nutrition Program between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Social Services,
commencing July 1, 2004, and ending June 30, 2005, with further terms and conditions being as
stated in said application, and
WHEREAS,after review,the Board deems it advisable to approve said application, a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,ex-officio Board of Social Services,that the Application for Community Food and
Nutrition Program between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Social Services, be, and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said application.
The above and foregoing Resolution was,on motion duly made and seconded, adopted by
the following vote on the 28th day of July, A.D., 2004, nunc pro tunc July 1, 2004.

RE: Community Food and Nutrition Program(C N) Application
Enclosed for Board approval is a Community Food and Nutrition(CFN) Application with the
Colorado Department of Local Affairs. The Application was discussed and reviewed at the
Board's Work Session of July 26, 2004.
The major provisions of the Application are as follows:
1. The funding period is July 1, 2004 through June 30,2005.
2. Weld County's request is to fund food baskets through the Weld Food Bank to serve all
or part of the waiting list that consists of over 325 seniors and disabled.

xx a: To coordinate existing private and public food assistance resources, whenever such coordination is
determined to be inadequate, to better serve low-income communities;
b: To assist low-income communities to identify potential sponsors of child nutrition programs and initiate
new programs in underserved and unserved areas; and
c: To develop innovative approaches at State and local levels to meet the nutrition needs of low-income
people.
Applicant Program Description: •
The current econarry continues to challenge those who don't have a solid resource base in addition to having other limiting factors
such as disabilities, seniors on a fixed inane or children. Currently there is a waiting list of over 325 seniors and disabled for
food. These are vulnerable citizens who have no where else to turn. Emergency Food Boxes are being distributed at record levels, the
highest in the history of the Weld Food Bank. CFN funding would be used to provide food asistance to the vulnerable in our colmmity.
