RE: APPROVE AMENDMENT#1 TO COLORADO WORKS AGREEMENT FOR SERVICES
AND AUTHORIZE CHAIR TO SIGN - ISLAND GROVE REGIONAL TREATMENT
CENTER
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Amendment#1 to the Colorado Works
Agreement for Services between the County of Weld,State of Colorado,by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and
Island Grove Regional Treatment Center,ending June 30,2004,with further terms and conditions
being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment,a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,ex-officio Board of Social Services,that Amendment#1 to the Colorado Works
Agreement for Services between the County of Weld,State of Colorado,by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and
Island Grove Regional Treatment Center be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be,and hereby is,authorized to
sign said amendment.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 12th day of April, A.D., 2004.

AMENDMENT TO COLORADO WORKS AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF SOCIAL SERVICES
AND ISLAND GROVE REGIONAL TREATMENT CENTER
This Amendment,made and entered into the_day of April 2004,by and between the Board of Weld
County Commissioners,sitting as the Board of Social Services, on behalf of the Weld County Department of Social
Services,hereinafter referred to as"Social Services,"and Island Grove Regional Treatment Center,hereinafter
referred to as"Island Grove."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement(TANF-03-0001)in order to increase its
funding and service; and
WHEREAS, Social Services has budgeted, appropriated,and otherwise made available; and
WHEREAS,required approval,clearance,and coordination have been accomplished from and with
appropriate agencies.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as follows:
1. Exhibit A,Reimbursement,Item B,will be revised as follows:
Total annual expenditures will not exceed$5,100.00.
2. Exhibit A,Reimbursement,Item D,will be added as follows:
SASSI forms costs will also be charged to the Department of Social Services.
3. Exhibit B,Payment Schedule,Funding and Method of Payment,will be revised as follows:
Social Services agrees to reimburse to Island Grove in consideration for the work and Services
performed,a total amount not to exceed Eighteen Thousand dollars($18,000.00)as outlined in
Exhibit A for screening and assessments and a total amount not to exceed Five Thousand One
Hundred dollars($5,100.00)for alcohol and drug education awareness.
This Amendment,together with the original Colorado Works Agreement for Services,constitutes the entire
understanding among the parties with respect to the subject matter hereof,and may not be changed or modified except as
stated in the Colorado Works Agreement of Servics.

RE: Amendment to Colorado Works Agreement for Services Between
the Weld County Department of Social Services and Island Grove
Regional Treatment Center, Inc.
Enclosed for Board approval is an Amendment to a Colorado Works Agreement for
Services between the Weld County Department of Social Services (Department) and
Island Grove Regional Treatment Center, Inc., (Island Grove). This Amendment was
reviewed at the Board's Work Session of March 29, 2004.
The major reasons for the Amendment are as follows:
A. Increase screening and assessments for Colorado Works Clients from $5,000 to
$5,100.
B. Increase total funding to Island Grove from $15,000 to $18,000 to cover alcohol
and drug treatment costs for Colorado Works clients.
All other terms of the Agreement remain the same including the term of the Agreement,
ending June 30, 2004.
