These grants are in response to needs data which shows that a high percentage of Weld seniors are lonely and isolated from community life. The senior centers listed below have agreed to double their efforts to reach out to these seniors by implementing telephone reassurance calls, one on one visitations, and buddy programs to attract new seniors to the centers and a variety of other outreach activities.
