RE: APPROVE TASK ORDER OPTION TO RENEW LETTER #3 FOR IMMUNIZATION
PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with Task Order Option to Renew Letter#3 for
Immunization Program between the County of Weld,State of Colorado,by and through the Board
of County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment,
commencing January 1, 2004, and ending December 31, 2004,with further terms and conditions
being as stated in said letter, and
WHEREAS, after review, the Board deems it advisable to approve said letter, a copy of
which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Task Order Option to Renew Letter #3 for Immunization Program
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be,and hereby is, authorized to
sign said letter.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 14th day of January, A.D., 2004, nunc pro tunc January 1, 2004.

Task Order Option to Renew Letter Number 03, Contract Routing Number 04 FHA 00178
State Fiscal Year 2004-2005, Immunization Proeram
This Task Order Option to Renew Letter is issued pursuant to paragraph F. of the Master Contract identified by
contract routing number 00 FAA 00008 and paragraph C.4. of the Task Order identified by contract routing
number 01 FAA 01014 and contract encumbrance number PO FAA IMM0101014. This Task Order Option to
Renew Letter is between the COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT
and BOARD OF COUNTY COMMISSIONERS OF WELD COUNTY. The Task Order has been amended by
Task Order Change Order Letter**.contract routing number****** *****,and/or Task Order Option to Renew
Letter 01 contract routing number 02 FAA 00827 and Task Order Option to Renew Letter 02,contract routing
number 03 FAA 00677 if any. The Task Order,as amended,if applicable,is referred to as the"Original Task
Order". This Task Order Option to Renew Letter is for the renewal term of January 1.2004 through December
31,2004. The maximum amount payable by the State for the work to be performed by the Contractor during this
renewal term is Forty-Seven Thousand Three Hundred Eighty-Seven Dollars ($47,387.00)for an amended
total financial obligation of the State of ONE HUNDRED FIFTY THOUSAND NINE HUNDRED SEVENTY-
FOUR DOLLARS ($150,974.00). This is an increase/decrease of($47,387.00)of the amount payable from the
previous term. The Budget for this renewal term is incorporated herein by this reference,made a part hereof,and
attached hereto as"Attachment A". The first sentence in paragraph C. of the Original Task Order is modified
accordingly. All other terms and conditions of the Original Task Order are reaffirmed. This Task Order Option to
Renew Letter is effective upon approval by the State Controller, or designee,or on January 1,2004 whichever is
later.
Please sign,date,and return all four(4)originals of this Task Order Option to Renew Letter by December 26,
2003 to the attention of: Erikka Apolinar,Colorado Department of Public Health and Environment,Mail
Code IMM-A4,4300 Cherry Creek Drive South,Denver,Colorado 80246-1530. One original of this Task
Order Option to Renew Letter will be returned to you when fully approved.

Dear Colleague:
Attached please find your 2004 Immunization Service Delivery contract. Currently,the federal budget has not
been approved. Therefore,The Centers for Disease Control and Prevention(CDC)is only able to fund states at
25%of their award level for 2004. As a result,the Immunization Program is not able to fully fund contracts at
their intended 2004 amounts at this time. The attached 2004 contract represents level funding from 2003. But
wait! As soon as we receive notification from CDC regarding the fulfillment of our funding request,we will
amend contracts to bring them up to their 2004 funding level. We anticipate receiving our fill award before
February 1,2004.
The fully funded contractual amounts for 2004 were determined by the Immunization Program based on a
formula that includes population and birth figures,poverty,WIC enrollment and the following:
• The change in births in each county from 2002 to 2003 (some increased,some decreased)
• The proportion of the state's non-white births in 2003 occurring in each county.
Based on this formula, local health agencies in areas that experienced a decrease in births will see a decrease in
their funding;conversely,those with an increase in births will see an increase in funding. We focused on births
instead of overall population changes because the bulk of immunization services are provided to infants and
young children.
The Immunization Program contract monitor will be contracting you in January 2004 to review the scope of work
outlined in this contract. If you have any questions,or would like to discuss your service delivery funding for

SUBJECT: Task Order Option to Renew Letter#3 for
the Immunization Program
Enclosed for Board review and approval is a task order option to renew letter between the
Colorado Department of Public Health and Environment (CDPHE) and Weld County for the
Immunization Program.
Under the provisions of this task order, Weld County will receive an amount not to exceed
$47,387 to provide community education regarding the rules of the Colorado Board of Health
pertaining to the immunization of students attending school and rules pertaining to the infant
immunization program. WCDPHE will also conduct reminder/recall activities, will collaborate
with the State's Supplemental Nutrition Program(WIC), will serve as a resource for the Weld
County Immunization Coalition, and will provide walk-in immunization clinics each month
for persons of all ages. The time period of this task order is from January 1, 2004 through
December 31, 2004.
I recommend your approval of this task order option to renew letter.
