RE: APPROVE OPTION TO EXERCISE RENEWAL OF PURCHASE OF SERVICES
AGREEMENT AND AUTHORIZE CHAIR TO SIGN - UNIVERSITY OF NORTHERN
COLORADO
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Option to Exercise Renewal of a
Purchase of Services Agreement between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County,on behalf of the Weld County Department of
Public Health and Environment,and the University of Northern Colorado,commencing January 1,
2004, and ending December 31, 2004, with further terms and conditions being as stated in said
option, and
WHEREAS, after review, the Board deems it advisable to approve said option, a copy of
which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Option to Exercise Renewal of Purchase of Services Agreement
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the University of Northern Colorado be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said option.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 28th day of January, A.D., 2004, nunc pro tunc January 1, 2004.

Level of service change in conjunction with renewal for additional term

For Testing for Detection of Chlamydia Trachomatis and Neisseria Gonorrhea
Contract Expires: Perpetual until terminated by either party
In accordance with Paragraphs 1 and 3 of the Purchase of Services Agreement under contract routing number GKA01-0040
between the State of Colorado acting by and through the Board of Trustees of the University of Northern Colorado,Auxiliary
Services, for the benefit of the Student Health Center(State), and Weld County Department of Public Health & Environment
(Contractor), the State hereby exercises the option for an additional one-year period to include January 1, 2004 through
December 31, 2004.
The maximum amount payable by the State to Contractor is $20.00 per student for the GenProbe Aptima Combo 2 testing
(for combination testing of Chlamydia and Neisseria gonorrhoeae). The amount of the current Fiscal Year contract value is
increased by$3.00 per test per student to satisfy services/goods ordered under the contract for FY 2003-2004/05.
The contract is hereby modified accordingly and pass-thru funds have been encumbered in Fund No. 2-3110.

CRS 24-30-202 requires that the State Controller approve all state contracts. This contract is not valid until the State
Controller, or such assistant as he may delegate, has signed it. The contractor is not authorized to begin performance until
the contract is signed and dated below. If performance begins prior to the date below, the State of Colorado may not be
obligated to pay for goods and/or services provided.

SUBJECT: Option to Exercise Renewal of Purchase
of Service Agreement with UNC
Enclosed for Board review and approval is an option to exercise renewal of the purchase of
services agreement with UNC for Chlamydia and Neisseria gonorrhoeae testing.
The agreement, which was originally approved by the Board of County Commissioners in
September 2000, allows Weld County Department of Public Health and Environment to provide
laboratory testing of medical samples for the UNC Student Health Center. Through this option
to exercise renewal, WCDPHE will continue to provide these services for the time period
January 1, 2004 through December 31, 2004 using the GenProbe Aptima Combo 2 test. UNC
will pay WCDPHE $20 per specimen tested for the services.
I recommend your approval of this option to exercise renewal.

Dear Ms. Coleman:
The Weld County Department of Public Health and Environment Laboratory currently has a
Purchase of Services Agreement with the University's Student Health Center to provide
Chlamydia and Neisseria gonorrhoeae testing. The current testing method used is a GenProbe
Aptima Combo 2 test. The current charge for this combination test is $ 17.00 per specimen.
Beginning January 1, 2004, the cost for this combination test will be increasing to $20.00 per
specimen.
