RE: APPROVE LEASE FOR OFFICE SPACE IN SOCIAL SERVICES BUILDING AND
AUTHORIZE CHAIR TO SIGN - MULTIDISCIPLINARY YOUTH ASSESSMENT
(MYAT) TEAM
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Lease for office space in the Social
Services Building between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Social Services, and
Multidisciplinary Youth Assessment Team, including the Weld County Division of Human
Services, Weld County Department of Public Health and Environment, the Island Grove Regional
Treatment Center, Inc., and North Range Behavioral Health, Inc., commencing June 1, 2004,
and ending June 30, 2006, with further terms and conditions being as stated in said lease, and
WHEREAS, after review, the Board deems it advisable to approve said lease, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the Lease for office space in
the Social Services Building between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Social
Services, and Multidisciplinary Youth Assessment Team, including the Weld County Division of
Human Services, Weld County Department of Public Health and Environment, the Island Grove
Regional Treatment Center, Inc., and North Range Behavioral Health, Inc., be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said lease.

RE: Lease Agreement Among Weld County D rtment of Social Services,
Weld County Division of Human Services, Weld County Department of
Public Health and Environment, Island Grove Regional Treatment
Center, Inc., and North Range Behavioral Health, Inc.
Enclosed for Board approval is a Lease Agreement among the Weld County Department of Social
Services (Department), Weld County Division of Human Services (Employment Services),Weld
County Depai lucent of Public Health and Environment(Public Health), Island Grove Regional
Treatment Center, Inc. (Island Grove), and North Range Behavioral Health, Inc., (North Range).
The Lease Agreement was reviewed at the Board's Work Sessions of July 26, 2004.
The major provisions of the Agreement are as follows:
1. The term period is from June 1, 2004 through June 30, 2006.
2. The Department agrees to provide space to the parties listed above at its 315 North 11`x'
Avenue, Greeley, location at no rent charge including costs associated with the
Multidisciplinary Youth Assessment Team(MYAT) Program.
3. Employment Services, Public Health, Island Grove, and North Range agree to provide a
FTE staff person, as defined in the MYAT Agreement, to provide MYAT services for
youth and families.

LEASE
THIS LEASE, made and entered into this,� day of U�U er 2004,by and between the COUNTY OF
WELD, a political subdivision of the State of Colorado,by and through the Board of County Commissioners of the
County of Weld, whose address is 915 10th Street,Greeley, Colorado 80631,hereinafter referred to as"Lessor,"and
the Weld County Division of Human Services, hereinafter referred to as "Human Services", the Weld County
Department of Public Health and Environment, hereinafter referred to as "Public Health", and sitting as the Board
of Social Services for the Weld County Department of Social Services, hereinafter referred to as"Social Services";
Island Grove Regional Treatment Center, Inc., hereinafter referred to as `Island Grove"; North Range Behavioral
Health, Inc., hereinafter referred to as "North Range", for the Multidisciplinary Youth Assessment Team (MYAT)
hereinafter referred to as"Lessee".
WITNESSETH:
That in consideration of the keeping and performance of the covenants and agreements by Lessee,
hereinafter set forth, Lessor hereby leases unto Lessee the following described premises, situated in the County of
Weld and in the State of Colorado, one cubicle per MYAT Contractor to-wit: located in the Weld County
Department of Social Services building at 315 North 11th Avenue, Greeley, Colorado, hereinafter referred to as"the
Premises."
Furthermore, the Lessee, in consideration of leasing of said premises as aforesaid, covenants and agrees as
follows:
a. There will be no rent charged to the Lessee by the Lessor for use of the aforesaid premises as herein
provided;
b. To use ordinary care when using the improvements upon the Premises;
c. To sublet no part of the Premises;
d. To use the Premises for no purpose other than conducting the business and duties as
a MYAT Contractor;
e. To keep the Premises clean,and in the sanitary condition required by the
Ordinances and the Health and Police Regulations of the City of Greeley and/or the County of Weld;
f. To allow Lessor at any reasonable hour of the day to enter into or upon and make
improvements to equipment therein.
Furthermore,the parties hereto agree as follows:
1. That all costs for utilities and trash removal shall be paid for by Lessor;
2. That no assent, express or implied, to any breach of any one or more of the covenants or
agreements hereof shall be deemed or taken to be a waiver or any succeeding or other breach;
3. That Lessee agrees to indemnify, save, and hold harmless Lessor from any and all loss, injury, or
damage whatsoever, incurred by Lessee as the result of fire, flood, inclement weather, or other
casualty,other than by negligence of the Lessor;
4. That Lessor shall own and maintain the filing cabinets and computer systems including
computers,modems,printers, and associated computer equipment;
5. MYAT Contractors agree to escort all MYAT clients to and from the reception area;
6. The Lessor shall pay for use of the telephones and all miscellaneous office supplies including
photocopying for purposes of MYAT functions only;
7. That the duration of the lease agreement shall commence on June 1, 2004,and terminate on
June 30,2006.

IT IS EXPRESSLY UNDERSTOOD AND AGREED by and between the parties herein, that it shall and
my be lawful for the Lessor to declare said term ended and enter into the Premises, or any part thereof, either with
or without process of law, to reenter, and Lessee or any person or persons occupying the same, to expel, remove,
and put out, using such force as may be necessary in doing so, without being liable to prosecution or any damage
therefor, and the Premises again to repossess and enjoy, is in the first and former estate of the Lessor and if at any
time said term shall be ended as aforesaid or in any other way,the Lessee hereby covenants and agrees to surrender
and deliver up the Premises peaceably to Lessor immediately upon the termination of said term, and if the Lessee
shall remain in possession of the same after the termination thereof, Lessee shall be deemed guilty of a forcible
detainer of the Premises under the statute, hereby waiving all notice, and shall be subject to eviction and removal,
forcibly or otherwise, with or without process of law,as above stated.
IT IS FURTHER EXPRESSLY UNDERSTOOD AND AGREED that all the covenants and agreements in
the Lease contained shall extend to and be binding upon the heirs,exactors, legal representatives, and assigns of the
respective parties hereto.
