to design and construct a two lane roadway that consists of 12'paved lanes, 10' shoulders(8'paved—2'gravel)for the
three mile section of gravel roadway with consideration for the ultimate section.

The following change is hereby made to the Contract Documents:
Include change of the Scope of Work to include pothole testing for utilities and prepare and conduct a public open
house meeting. (See attached support documentation)
Add additional calendar days due to rime lost for County review and public review and input of preliminary plans.

CHANGE TO CONTRACT PRICE:
Original Contract Price: $ 218,398.00
Current Contract Price adjusted by previous Change Order: $ 218,398.00
The Contract Price due to this Change Order will be increased by: $ 8,616.00
The new Contract Price,including this Change Order,will be$ 227,014.00
CHANGE TO CONTRACT TIME:
The Contract time will be increased by 111 calendar days.
The date for completion of all Design Work will be March 21,2004 .

Dear Wayne:
This letter is address the change of scope for including a public meeting scheduled for
December 11, 2003 as part of our contract. Van Thuyne-Downs, Inc. completed
potholing activities and the invoice is included in Change Order#1. I also am submitting
documentation for days lost to review, and estimated time to complete the design portion
of the work.
• Change Order#1
Jehn Engineering provided materials and personnel necessary to hold a public
meeting on December 11, 2003 at the City of Dacono. Jehn Engineering prepared
exhibits suitable for public discussion, which included the aerial view with the
proposed 140' ROW, the interim section, existing and proposed utilities, and existing
and proposed profiles. 2 sets of 11" X 17" construction plans were available for
reference, specifically the ownership maps and cross sections. Additionally, Jehn
Engineering prepared a "Fact Sheet" at the request of Keith Meyer using an example
he had forwarded. The following is a breakdown of Change Order# 1.
Open House
Senior Project Manager 8 hours @ $100.00/hour= $800.00
Senior Technician 12 hours @ $68.00/hour= $816.00
Senior Technician (Fact Sheet) 10 hours @ $68.00/hour= $680.00
Clerical 6 hours @ $50.00/hour = $300.00
Reimbursables Exhibits $500.00
Total Open House $3,096.00

This change order shall conform to the terms and conditions of the agreed upon
Contract.
• Jehn Engineering is requesting 69 days lost to review to be credited to the schedule.
Jehn Engineering had submitted the alignment shift exhibits and Bull Canal
hydraulics evaluation on 10/07/03 and we received feedback at our scheduling
meeting on October 14, 2003. Jehn Engineering submitted the F.I.R. plans on
October 21, 2003 and we received comments at the F.I.R. meeting on October 28,
2003. The time lost for determining the interim section and potential changes in
alignment began on November 12, 2003 with the recommendation of an interim
section and should continue until January 13, 2004. Please let me know if you do not
agree with the requested days lost.
• Jehn Engineering estimates the time to complete the design portion of the project to
be 42 days once we receive feedback from the January 13, 2004 follow-up meeting
with the property owners. We estimate 14 days to revise the F.I.R. plans and 28 days
to complete the F.O.R. plans, exclusive of additional time lost for review.

Please submit the enclosed item on the signature agenda.
Change Order No. 1 for Public Works Project WCR 13, from WCR 2 to WCR.8.
Change Order No. 1 is to revise the contract between Weld County and Jehn Engineering,Inc. for the
design and construction inspection of WCR 13, from WCR 2 to WCR 8. The Change Order includes
an increase to the project cost for additional design work requested by the Public Works Department.
Additional contract time to perform the extra work and time lost for plan review by the County will
be included in the Change Order.
The appropriate documentation is enclosed.
