Request for Proposal
I. Project Description
Catholic Charities'Promoting Safe and Stable Families (PSSF)program continues to work to
strengthen Weld County families. PSSF promotes safety, permanency and well-being for
client children through the provision of flexible, strengths-based, family-centered support all
the while maximizing community collaboration. Catholic Charities' PSSF program is unique
in its family-case manager partnership that cultivates sustainable strengths, navigates systems
(Schools, legal, mental health, social/human service) and improves access to and the
coordination of services. Weld County will not be financially accountable for these services,
as Catholic Charities is able to provide this program through grant funding.
II. Target/Eligibility Populations
Catholic Charities' PSSF focuses on preservation and support services for families where a
child(ren) is at imminent risk for removal, or who has been removed from the family setting
and placed in out-of-home care is preparing to or has recently transitioned home. Ideally, the
reunification occurs within 15 months from the date of placement. Referrals will be made by
the Department of Social Services to Catholic Charities at the point of intake or when the
ongoing unit is planning reunification and closing the case. The referrals will be voluntary
only.

referrals from Weld County. The Program has the capacity to serve 40
families from October 1, 2004 to September 30, 2005.

G. The monthly average capacity: 12 families
H. Average stay in the program: 90 days
I. Average hours per week in the program: 3-4 hours per week.

the case manager, utilization of the program on a weekly basis will vary.
