RE: APPROVE APPLICATION FOR PREVENTIVE HEALTH AND HUMAN SERVICES
BLOCK GRANT AND LETTER OF SUPPORT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Application for the Preventive Health
and Human Services Block Grant and Letter of Support from the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Department of Public Health and Environment, to the Colorado Department of
Public Health and Environment, commencing October 1, 2004, and ending September 30, 2005,
and
WHEREAS, after review, the Board deems it advisable to approve said application, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Application and Letter of Support for the Preventive Health and
Human Services Block Grant from the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Department of
Public Health and Environment, to the Colorado Department of Public Health and Environment
be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said application and letter of support.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 20th day of September, A.D., 2004.

Enclosed for Board review and approval is the application for the
Preventive Health Block Grant. The contract would be from October 1,
2004 to September 30, 2005. The grant request is for $16,365.24
Weld County Department of Public Health and Environment will use this
funding to provide education to physicians' office staff and clinic staff about
a variety of immunization topics and to co-host at least three Spanish
language radio health talk shows about immunizations. This should help to
increase the number of children in Weld County who have an adequate
number of immunizations.

Preventive Health and Human Services Block Grant Application 2004-2005
Weld County Colorado
1) Program/Project Title: Community Provider Education
2) Program/Project Strategy: During 2003-2004 the Preventive Health Block Grant
helped establish the Alliance for Latino Health and Education of Weld County. This
group has met monthly during the grant year. The leaders of the Alliance have worked to
increase the immunization rates in Weld County by assisting at community events where
immunizations were offered, such as the Children's Festival and ajoint car
seat/immunization day in September. Health education has also been provided to the
Spanish speaking population through half-hour talk shows co-hosted by health
professionals on a local Spanish language radio station on Saturdays at 1 p.m. The
Alliance will continue to meet and give direction to health and educational outreach
projects in Weld County.
For the 2004-2005 Preventive Health Block Grant, we propose a project that will
address the health care providers in Weld County. A public health nurse who coordinates
the immunization program at Weld County Department of Public Health and
Environment will provide four in-service trainings about immunizations for office staff in
physicians' offices and clinics in at least four sites throughout the county during the grant
year. The trainings will be held during lunch hours, with pharmaceutical companies
providing the food and beverages.
Topics that will be covered during these in-services include how to read the
immunization schedule, why some shots are given intermuscular(IM) and others are
subcutaneous (SQ), how to address parental concerns about the safety of immunizations,
proper storage of vaccines, new combination vaccines, reactions and allergies to
vaccines, etc. Each attendee will receive a three-ring binder to hold handouts from each
in-service for easy reference, which will result in a resource binder about immunizations
at the end of the year. This binder will also contain contact information for staff at the
Health Department, as well as websites for additional information about vaccines and
immunizations. Books addressing some of the related issues will also be purchased and
given for references to each physicians' office and/or clinic that is represented at the in-
services.
Due to the frequent staff turnover in physicians' offices and clinics, many
employees will benefit from these educational events. These in-services should help
reduce the number of frequently missed opportunities for immunizations at offices and
clinics. The end result will be improved immunization rates for Weld County children.
In addition, a portion of the grant funds will be used to aid in the continuation of
the successful health talk show on the local Spanish language radio station on Saturdays
at 1 p.m. A bilingual health professional will co-host the programs and present
information about the importance of immunizations, highlight immunization events and
answer questions called in by listeners.
3) National Health Objective(s): The following objectives from the Healthy People
2010 health objectives will be addressed by this project:
14-1: Reduce or eliminate indigenous cases of vaccine-preventable diseases
14.22: Achieve and maintain effective vaccination coverage levels for universally
recommended vaccines among young children.
14.24: Increase the proportion of young children who receive all vaccines that have been
recommended for universal administration for at least 5 years.
14.25: Increase the proportion of providers who have measured the vaccination coverage
levels among children in their practice population within the past two years.
4) State Health Objective(s)—
5) Health Problem: Currently, only 67.2% of children in Weld County under the age of
three have received the recommended number of vaccinations. This low rate of
immunizations for vaccine preventable diseases place entire communities at a higher risk.
6) Target Population: The direct target population will be staff meinbers who work in
eight to ten physician offices and clinics that treat children in Weld County. The indirect
population will be the children who receive their care at these clinics and offices. The
Spanish speaking residents of Weld County and surrounding counties are the target for
the health talk radio programs.
7) Disparate Population: 42,000 (20.4%) of Weld County residents live in a family
with an income below 200% of poverty level. 26,263 (12.7%) of Weld County residents
were without health insurance in 2003. The Vaccine for Children(VFC)program is
available to provide immunizations for many of these children. This educational program
will make office staff aware of VFC and other services available to this medically
underserved population.
The Weld County Department of Public Health and Environment currently has a very
successful partnership with the local Spanish language radio station. Bilingual health
professionals co-host the half-hour radio talk shows during prime time, Saturdays at 1
p.m. A variety of topics have been covered: diabetes, back to school health
preparedness, food safety, West Nile virus, etc. The number of listeners and called-in
questions has increased markedly during the four months the program has been in
existence. A small amount of funding has been included in this grant to be able to
continue this programming. Bilingual public health nurses will co-host the radio program
at least three times during the next year to explain the importance of immunizations,
highlight immunization events, and answer call-in questions from listeners.
8) Essential Service Areas:

• Impact Objective—By 2010 90% of Weld County children under the age of 3
will have up-to-date immunization records
• Annual Objective—By October 2005 at least four immunization in-services will
be provided to eight to ten doctors' office and clinic staff. (a total of at least 16)
and at least three Spanish language radio programs about immunizations will be
presented.
9) Local Support—None of the Preventive Health and Human Services Grant funding
will be distributed to other local agencies
10) FTE - 0.2 FTE for a public health nurse will be funded with these dollars.
11) Block Grant Role—

13) New Request/Renewal - This is the second year that PHHS funds have been
requested for an immunization project. This is a decrease in funding from the previous
year's grant due to a re-direction in programming and project objectives. The project
funded last year(establishment of a Latino Health Alliance) is up and running and will be
maintained through other income sources.
14) Other funding sources —County funds will be used to pay the salaries of other staff
members (office technician and supervisors)who will work on this project. ($9,516.37)
Pharmaceutical companies will also help fund this project when they provide the food
and beverages for the participants during the lunch-time trainings. (estimated: $2,400)
Other grants and funding help pay for most of the health talk show time slot, as well as
small stipends for bilingual health professionals from the community who serve as co-
hosts to discuss their area of expertise.

The Weld County Board of Commissioners is pleased to provide this letter of support for
the application for the Preventive Health and Health Services Block Grant made by the Weld
County Department of Public Health and Environment. The amount of the request is for
$16,365.24 for the 2004-2005 funding cycle. The Health Department has effectively used these
funds in previous years for a wide range of programs in our community.
The Preventive Health and Health Services Block Grant funds will be used to provide
education about immunizations to community health providers in Weld County. This will be
accomplished by a series of trainings held in several locations for physicians' office staff and
clinic staff members. During the course of the trainings, participants will learn about and create
resource books with information about safe vaccine storage, the immunization schedule, how to
address parental concerns regarding the safety of immunizations, allergies and reactions to
vaccines, etc This should help prevent missed opportunities for needed immunizations.
A small part of the grant funding will also be used to continue the successful weekly
health talk program on the local Spanish language radio station. Radio has proven to be an
effective way to reach the Spanish speaking population of the county, and will be used to
educate Latinos about required immunizations and available resources.
The Health Department has always been a leader in the implementation of new and
innovative strategies to protect the health of the residents of Weld County. The Board of
County Commissioners supports the Weld County Department of Public Health and
Environment in its goal to improve childhood immunization rates in Weld County.
