RE: APPROVE AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
AND AUTHORIZE CHAIR TO SIGN - SIGNAL BEHAVIORAL HEALTH NETWORK
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Child Protection
Agreement for Services between the County of Weld,State of Colorado,by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and
Signal Behavioral Health Network,commencing upon full execution of said amendment,and ending
May 31, 2004, with further terms and conditions being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment,a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,ex-officio Board of Social Services,that the Amendment to the Child Protection
Agreement for Services between the County of Weld,State of Colorado,by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and
Signal Behavioral Health Network be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be,and hereby is,authorized to
sign said amendment.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 18th day of February, A.D., 2004.

AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF SOCIAL SERVICES
AND SIGNAL BEHAVIORAL HEALTH NETWORK
This Amendment, made and entered into the 18day February 2004by and between the Board of
Weld County Commissioners, sitting as the Board of Social Services, on behalf of the Weld County Department of
Social Services,hereinafter referred to as"Social Services," and Signal Behavioral Health Network,referred to as
"Signal". cl` `.
WITNESSETH %r
WHEREAS,the parties desire to modify their current Agreement(PY 03/04-CPS-30) in order to increase
its funding and service; and
WHEREAS, Social Services has budgeted,appropriated,and otherwise made available;and
WHEREAS, required approval,clearance, and coordination have been accomplished from and with
appropriate agencies.
NOW THEREFORE, in consideration of the premises,the parties hereto covenant and agree as follows:
I. Exhibit B, Payment Schedule, Item I. Funding and Method of Payment.!.vill be revised as
follows:
Social Services agrees to reimburse Signal in consideration for the work and services performed, a
total amount not to exceed One Hundred-Seventy Two Thousand, Three Hundred Ninety-eight
Dollars($172,398.00) under Program Code 1050 and Function Code 1850. Of this total amount,
Social Services agrees to pay an administrative fee not to exceed Fourteen Thousand, Two
Hundred Thirty-nine Dollars and Eighty Cents($14,239.80)or 10%of Fifty Two Thousand Three
Hundred Ninety-Eight Dollars ($52,398) in work and services performed and 7.5%administration
fee for One Hundred Twenty Thousand Dollars($120,000)in work and services performed.
2. Exhibit C,Core Services Fee Schedule,Other Services,will be revised as follows:
Tox Trap Tests(Includes confirmation)at$40.00 per test
UA Test at$12.00 per test,excluding UA confirmation
This Amendment, together with the original Child Protection Agreement for Services,constitutes the entire
understanding among the parties with respect to the subject matter hereof, and may not be changed or modified
except as stated in the Child Protection Agreement of Services.

RE: Amendment to Child Protection Agreement for Services Between the
Weld County Department of Social Services and Signal Behavioral
Health Network
Enclosed for Board approval is an Amendment to Child Protection Agreement for Services
between the Weld County Department of Social Services(Depai lucent)and Signal Behavioral
Health Network(Signal). The Amendment was discussed and reviewed at the Board's Work
Session of December 29,2003.
The major provisions of the Amendment are as follows:
1. The Department agrees to increase the funding level of the Agreement to $172,398. This
is an increase of$120,000.
2. Signal agrees to decrease its administrative fee to 7.5% or$9,000, which would be
applied to the$120,000 increased funding level.
3. Signal agrees to decrease its cost of a Urinalysis(UA)test to $12.00 and add a test, Toxic
Trap Test, at a cost of$40.00 per test.
All other terms of the Agreement remain the same including the term of the Agreement, ending
May 31, 2004.
