RE: APPROVE CONSENT TO ASSIGNMENT AND RELEASE (FUNDING SOURCES) FOR
SERVICES AND AUTHORIZE CHAIR TO SIGN - COLORADO CHRISTIAN HOMES
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Consent to Assignment and Release
(Funding Sources)from Colorado Christian Homes to Friends of Children,by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, with
further terms and conditions being as stated in said consent, and
WHEREAS, after review,the Board deems it advisable to approve said consent, a copy of
which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,ex-officio Board of Social Services,that the Consent to Assignment and Release
(Funding Sources), from Colorado Christian Homes to Friends of Children, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Social Services,
be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said consent.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 13th day of October, A.D., 2004.

RE: Consent to Assignment and Release (Funding Sources)—' riends of
Children
Enclosed for Board approval is a Consent to Assignment and Release (Funding Sources)with
Friends of Children. This Consent was reviewed at the Board's Work Session of October 4,
2004. Bruce Barker, Weld County Attorney, has reviewed and approved signing the Consent.
The Department entered into three Agreements with Colorado Christian Homes for services under
Child Placement Agency(CPA), Residential Treatment Center(RTC), and Residential Child
Care Facilities(RCCF). The Colorado Christian Home is being acquired by a new Colorado non-
profit organization—Friends of Children. We recommend the transfer of these contractual
responsibilities from Colorado Christian Homes to Friends of Children and request your signature
on the Consent to Assignment and Release.

CONSENT TO ASSIGNMENT AND RELEASE (FUNDING SOURCES)
The undersigned, County of Weld , as
a party under two Agreements dated 07/14/2004 , hereby consents to the
assignment of the rights and obligations of Colorado Christian Home as a party under said
contract to Friends of Children, assignee, and also hereby releases Colorado Christian Home
from any further obligations under said contract.
