WELD COUNTY, COLORADO

The Board of County Commissioners of Weld County, Colorado, met in regular session as the Board of
Social Services in full conformity with the laws of the State of Colorado at the regular place of meeting in
the Weld County Centennial Center, Greeley, Colorado, July 7, 2004, at the hour of 9:00 a.m.
ROLL CALL: The meeting was called to order by the Chair and on roll call the following members were
present, constituting a quorum of the members thereof:
Commissioner Robert D. Masden, Chair
Commissioner William H. Jerke, Pro-Tern
Commissioner M. J. Geile
Commissioner David E. Long
Commissioner Glenn Vaad
Also present:
County Attorney, Bruce T. Barker
Acting Clerk to the Board, Esther E. Gesick
Director of Finance and Administration, Don Warden
MINUTES: Commissioner Long moved to approve the minutes of the Board of Social Services meeting
of June 28, 2004, as printed. Commissioner Jerke seconded the motion, and it carried unanimously.
WARRANTS: Don Warden, Director of Finance and Administration, presented the warrants for approval
by the Board in the amount of $417,276.77. Commissioner Jerke moved to approve the warrants as
presented by Mr. Warden. Commissioner Vaad seconded the motion, which carried unanimously.
NEW BUSINESS:
CONSIDER COLORADO WORKS PROGRAM AGREEMENT FOR SERVICES AMONG VARIOUS
ENTITIES AND AUTHORIZE CHAIR TO SIGN: Judy Griego, Director of Department of Social Services,
stated she recommends approval of the Colorado Works Program Agreement among North Range
Behavioral Health, Island Grove Regional Treatment Center, Inc., Department of Human Services, and
Public Health and Environment. Ms. Griego stated under this agreement the contractors will continue to
provide Multidisciplinary Youth Assessment Team (MYAT)and system-wide development to benefit the
positive development of youth and the strengthening of families under a two-year phase-in plan. For these
services the Department of Social Services agrees to pay a total of$208,000 in start up fees to be paid prior
to June 30,2004. Commissioner Vaad moved to approve said agreement and authorize the Chair to sign.
Seconded by Commissioner Long, the motion carried unanimously.
CONSIDER CHILD PROTECTION AGREEMENT FOR SERVICES AND AUTHORIZE CHAIR TO SIGN-
YUMIL JIMENEZ:Ms.Griego stated under this agreement Yumil Jimenez will provide home family therapy,
individual therapy, and consultation/education for a child placed out of the County. She stated for these
services the Department of Social Services agrees to reimburse the service provider$60.00 per hour,for

a term commencing April 12,2004,and ending December 31,2004. In response to Commissioner Jerke,
Ms. Griego stated there is a payment cap,and the child is in Larimer County. Commissioner Vaad moved
to approve said agreement and authorize the Chair to sign. The motion was seconded by Commissioner
Long, and it carried unanimously.
Let the minutes reflect that the above and foregoing actions were attested to and respectfully submitted by
the Acting Clerk to the Board.
There being no further business, this meeting was adjourned at 9:10 a.m.
