BOARD OF SOCIAL SERVICES
WELD COUNTY, COLORADO

The Board of County Commissioners of Weld County, Colorado, met in regular session as the Board of
Social Services in full conformity with the laws of the State of Colorado at the regular place of meeting in
the Weld County Centennial Center, Greeley, Colorado, December 20, 2004, at the hour of 9:00 a.m.
ROLL CALL: The meeting was called to order by the Chair and on roll call the following members were
present, constituting a quorum of the members thereof:
Commissioner Robert D. Masden, Chair
Commissioner William H. Jerke, Pro-Tem
Commissioner M. J. Geile
Commissioner David E. Long
Commissioner Glenn Vaad
Also present:
County Attorney, Bruce T. Barker
Acting Clerk to the Board, Debra S. Miller
Director of Finance and Administration, Don Warden
MINUTES: Commissioner Geile moved to approve the minutes of the Board of Social Services meeting
of December 15,2004, as printed. Commissioner Long seconded the motion,and it carried unanimously.
WARRANTS: Don Warden, Director of Finance and Administration, presented the warrants for approval
by the Board in the amount of $37,557.08. Commissioner Long moved to approve the warrants as
presented by Mr. Warden. Commissioner Jerke seconded the motion, which carried unanimously.
NEW BUSINESS:
CONSIDER AGREEMENT FOR RECREATIONAL/EDUCATION SERVICES FOR COLORADO WORKS
SUMMER ACTIVITIES PROGRAM AND AUTHORIZE CHAIR TO SIGN: - TOWN OF LASALLE: Judy
Griego, Director, Department of Social Services, stated the agreement term is May 1, 2004, through
December 31, 2004. Ms. Griego stated data entry for the Summer Activities Program has not been
successful in the Colorado Benefit Management System and, to expedite payment, said agreement has
been established to pay providers. She explained the Town of LaSalle will be reimbursed at a rate of$125
for providing recreational or educational services to children and youth referred by the Department of Social
Services. Responding to CommissionerJerke, Ms.Griego stated the Town of LaSalle had only one or two
children that worked in the recreation department. Commissioner Vaad moved to approve said agreement.
Seconded by Commissioner Long, the motion carried unanimously.
CONSIDER THREE AGREEMENTS TO PURCHASE RESIDENTIAL TREATMENT CENTER SERVICES
WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN: Ms.Griego stated the agreementterm
is November 2, 2004, through June 30, 2005. Commissioner Long moved to approve said agreements.
Seconded by Commissioner Geile, the motion carried unanimously.

CONSIDER THREE AGREEMENTS TO PURCHASE RESIDENTIAL CHILD CARE FACILITIES SERVICES
WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN: Ms.Griego stated the agreement term
is November 2, 2004, through June 30, 2005, and the services include room, board and treatment.
Commissioner Vaad moved to approve said agreements. Seconded by Commissioner Jerke,the motion
carried unanimously.
CONSIDER GENERAL ASSISTANCE AGREEMENT FOR SERVICES AND AUTHORIZE CHAIR TO SIGN-
WELD COUNTY DENTAL SOCIETY: Ms. Griego stated the yearly agreement provides for emergency
dental care at a rate of $165 per person, per year, with a ten percent co-pay requirement by the
patient/eligible person. She stated the Department of Social Services will reimburse Weld County Dental
Society a maximum of$17,000 per year from Weld County General Assistance funds. Commissioner Long
moved to approve said agreement. Seconded by Commissioner Jerke, the motion carried unanimously.
CONSIDER CHILD PROTECTION AGREEMENT FOR SERVICES AND AUTHORIZE CHAIR TO SIGN-
CHILD ADVOCACY RESOURCE AND EDUCATION, INC.: Ms.Griego stated the yearly agreement is for
parent education,community education and children's programs,along with prevention training. She stated
the provider will be reimbursed a maximum of$49,494. Responding to Commissioner Vaad, Ms. Griego
stated the prevention training will involve 11,000 children in classroom settings. Commissioner Vaad moved
to approve said agreement. Seconded by Commissioner Geile, the motion carried unanimously.
CONSIDER REVISIONS TO OPERATIONS MANUAL, SECTION 2.000, SOCIAL SERVICES DIVISION
POLICIES AND PROCEDURES: Ms. Griego stated the revisions reflect a 2.5 percent cost of living
adjustment for Calendar Year 2005 for the basic maintenance rate for Weld County Certified Foster Care
Homes. She stated the source of funding is the Child Welfare Block Grant. Commissioner Vaad moved
to approve said revisions. Seconded by Commissioner Long, the motion carried unanimously.
Let the minutes reflect that the above and foregoing actions were attested to and respectfully submitted by
the Acting Clerk to the Board.
There being no further business, this meeting was adjourned at 9:20 a.m.
