This grant will continue tobacco use prevention work in collaboration with the Tobacco Free Weld County Coalition made up of local Weld County Citizens and other regional and statewide partners. The tobacco program grant funds will provide for salaries and benefits for three Weld County Department of Public Health and Environment employees for a total of 2.0 FTE. The grant funds will also provide funding to continue coordinating a tobacco use prevention and cessation program for Weld County. The program will address the following 4 goals: 1)maintaining program infrastructure, 2)reducing initiation of tobacco use by children and youth, 3)promote cessation of tobacco use among youth and adults and 4)decrease exposure to secondhand smoke. Additionally, activities under each of these 3 goals areas will focus on eliminating disparities in the county. Describe the experience of the applicant. Include work with the target community and in providing tobacco education, cessation and prevention services. This section is detailed in 2003-04 grant year application. 2. Provide a description of the community's overall population and identified priority populations. With 4,004 square miles, Weld County has an area greater than that of Rhode Island, Delaware, and the District of Columbia combined, and is the 3`d largest county in Colorado. It is bordered on the north by Wyoming and Nebraska and on the south by the Denver metropolitan area. Weld County has a population of approximately 207,120 and is estimated to exceed 250,000 by 2010 making it one of the most populous counties in the state. Weld County's population increased 13.4%between 2000 and 2002 and is the second fasted growing area in the state. Weld County is fast becoming recognized as an affordable county to live in by many people who work in Denver, Boulder, Fort Collins and other front-range communities. There are 26 incorporated cities or towns in Weld County. The county seat and principal city, Greeley, is located in the west central part of the county with more than 82,000 people calling it home. Generally, most of the remaining population resides within a 20-30 mile radius of Greeley. Due to an increased need for services in the southern portion of the county, a south county health department building opened in 2001 to serve the population in that part of the county. The backbone of the county's economy is farming and raising cattle. Weld County is home of several large cattle feeding operations that are among the nation's largest. Swift, formerly Monfort of Colorado is the largest and employs 4,000 people in Weld County. Because the county has a large agricultural base, there is a large influx of low income Latino/Hispanic migrant farm workers, which are one of the priority populations identified in the five-year strategic tobacco prevention plan for Weld County. Efforts to reach out to the Latino/Hispanic population continue to include the targeted distribution and placement of bilingual educational materials and media on smoking related health issues, SHS exposure hazards and benefits of quitting. The population of the county is 81% Caucasian, with small percentages (under 1%) of Black or African American, American Indian, and Alaska native, Asian, and Native Hawaiian or other Pacific Islander. Persons reporting Hispanic or Latino origin on the 2000 census represent 27% 2 of the population in Weld County. Greeley's race and ethnic make-up is 29.5% Hispanic and 70.5% non-Hispanic. With nearly 30% of the population being Latino/Hispanic a key effort of the five-year strategic tobacco prevention plan for Weld County has been to develop and maintain a number of activities that will reach this population. In Weld County approximately 70% of the population is younger than 44 years old, with 33% of this group who are age 20 or younger. One reason Weld County has such a young population is the two colleges with over 17,800 college students who attend either the University of Northern Colorado (UNC) or AIMS Community College on a yearly basis. Additionally in Grades K-12 there are 31,000 students (12,000 who are of Latino/Hispanic origin) who attend one of the 13 school districts scattered throughout the county. The large college population and K-12 populations as identified above are identified as priority populations for the Tobacco Free Weld County Program. 3. Describe the plan for hiring and retaining qualified staff that represent the community's racial and cultural diversity. List the personnel that will be dedicated to the grant. Tobacco Free Weld County Program existing staff members have been employees since 2001. In the 2004-05 project plan, The program coordinator, Megan Martinez is a candidate for a Masters in Public Health with an anticipated graduation of August 2004. TFWCP is requesting to hire one additional staff member as a part-time health education specialist. This person will be recruited and hired based on Weld County's hiring practices. A Spanish-English bilingual individual will be sought, but not required. Penny Duell, office technician has been an integral part of the tobacco team and brings many skills to the program. In addition to growth, the coalition was instrumental in passing an ordinance to ban smoking in all public places and workplaces in the city of Greeley in 2003. Coalition members also are involved in school based initiatives with staff members and cessation efforts. There are currently two coalition members trained who implement a local tobacco cessation class for adults. 6. Describe or list community partners and coalition members (organizations) who will support the implementation of your project plans. The Tobacco Free Weld County Program has a number of major strengths that help to highlight it as a well-established and effective tobacco prevention program capable of continued tobacco prevention and reduction efforts. Major strengths include a well-established coalition and community partnerships that help in forwarding the work of the TFWCP. The program also has bilingual English/Spanish capabilities for working with the monolingual Spanish-speaking residents in the County. The Weld County Department of Public Health & Environment also provides full support to the program. The TFWCP staff members reside in and are familiar with Weld County. Staff includes a Program Coordinator who is in the process of obtaining a master's level degree in the field of Public Health (MPH) and has past experience in tobacco prevention and one Tobacco Office Technician with extensive office and public experience who provides program and coalition support. As previously mentioned an additional staff member is being requested. The internal infrastructure of WCDPHE, which was established in 1938, provides the ideal location for the TFWCP. Both currently and historically the WCDPHE has managed a number of public health grants. It has been providing management support for TFWCP grants since 1991. Monthly fiscal reporting is one of the infrastructure duties that WCDPHE has streamlined. TFWCP staff members are familiar with and trained in developing and meeting monthly/annual program progress reporting and monitoring requirements. Computer software, workstations, Internet access and technical support are available to each staff member. Meeting facilities are available onsite in Greeley and additionally at the South County health department location. The WCDPHE also provides a broad array of other health professionals including the director who is a medical doctor, nurses, health educators, dieticians, environmental health experts and assorted administrative support personnel who are willing to provide program and technical support on an ongoing basis. The TFWCP has past experience in organizing community members to assist in changing or implementing health related policies and a history of assessing, educating, implementing and evaluating tobacco programming since the early 1990's. The TFWCP as mentioned has a diverse coalition of community members with a broad assortment of backgrounds and abilities. This coalition has a record of being politically active in decreasing public exposure to environmental tobacco smoke and reducing youth access to tobacco products. Consisting of many motivated citizens, they have accomplished a number of 5 policy related changes in the community. These changes consist of passing an ordinance to ban smoking in all public places and workplaces in Greeley in 2003; supporting a resolution that banned smoking within 50 feet of all county buildings and vehicles; development of tobacco vending machine restrictions and a number of small community youth access ordinances. Two positions are currently filled. One is a 1.0 FTE Health Education Specialist II/Program Coordinator, and the other is a .50 FTE Office Technician III. A third position is requested that is a .50 FTE Health Education Specialist I. The total personal costs 76,700 X 12 months plus 31.97% fringe benefits are 101,221.00 for FY 2004-05. Direct Operating Expenses Printing/Copying ($1,327.00) —Printing costs include approximately 500 printed materials at 2.00 per item for coalition brochures, cessation brochures and other needed program materials for a total of 1,000.00. Copying costs include approximately 5,450 copies of various materials for all projects and general office needs at .06 per copy for a total of 327.00. Postage/Shipping ($1,432.00) —Postage is a set rate for each program in the division at a monthly rate of 113.00 for 12 months for a total of 1,356.00. Shipping costs are estimated on shipping costs of 2.00 each for 45 items. The total for shipping costs is 90.00 for shipping brochures, certificates, videos and other educational materials as needed. Rent/Utilities ($0)—No funds are requested in this line item. Telephone/Fax/Internet access ($0) —No funds are requested in this line item. Office Supplies ($2,457.00)-Stationery and envelopes for general office needs and projects at 22.50 per item for approximately 11 items totals 247.50. Computer/printer supplies for ink cartridges and needed supplies for 5 items at 94.00 per item totals 470.00. General office supplies are any other materials used by the tobacco program and is prorated monthly at 145.00 per month for a total of 1,740.00. Meeting Expenses ($3,675.00) — Funds are requested for monthly coalition meetings expenses at 100.00 each for a total of 1,200.00. Additional meeting expenses are requested for meetings in other communities and with health care and educational professionals throughout the county at 75.00 per meeting for approximately 33 meetings for a total of 2,475.00. 20 Media Expenses ($9,633.00) —The program depends on media for all projects throughout the county. There are no television markets available other than Denver markets so the program depends on newspapers for access to the public. Based on averages for advertising in the previous year and expected ads in the next fiscal year, advertising is estimated as follows. 31.00 for 3 advertisements for a total of 93.00 for classified advertising of an open position, 90.00 for 6 ads for a total of 540.00 for high school newspapers advertisements and 200.00 per ad for 45 ads for a total of 9000.00 for general advertising in Weld County newspapers. Staff Development/Registrations ($600.00) —The costs for two staff members to attend the national TUPTI (Tobacco Use Prevention Training Institute) conference based on 2003 cost to register is 300.00 per person for a total of 600.00. Each staff member attending will register for different tracks in order to obtain the most information to share with other staff members and the coalition members. Staff Development/Memberships/Subscriptions ($150.00) —The funds requested will maintain memberships in GASP (Group to Alleviate Smoking Pollution), CTEPA (Colorado Tobacco Education and Prevention Alliance) and Greeley Latina Women. All three memberships are opportunities for relationship building in tobacco control and in the community. Educational Materials ($3,600.00)— Funds requested will provide for approximately 1,400 pieces of educational materials at a cost of 2.00 per piece. The number of pieces is divided equally among the project activities with an additional number specified in the basic infrastructure of the program. Educational materials include items such as brochures, videos, posters, tobacco related incentives and other items used in youth prevention, youth and adult cessation, youth access to tobacco and secondhand smoke issues. Travel In-State Travel ($2,590.00) —The amount requested is based on .360 cents per mile which is the county limit for mileage (.355)rounded to the nearest dollar with an estimated 2945 miles for a total of 1,060.20. Additional funds are requested for in-state travel for meetings and trainings as required by the state for lodging and Per Diem. The costs are estimated based on three staff members attending one meeting in Denver and two staff members attending one additional meeting or training in the Denver area. The amount requested for lodging is estimated on 150.00 per night for two night's hotel for a total of 300.00. The per diem is estimated at 35.00 per day per staff member for a total of 280.00. Out of State Travel ($2,029.00)—The out of state travel requested is for two staff members attending the TUPTI conference. The costs are estimated for air travel at 300.00 per person for a total of 600.00, additional costs include airport parking and airport transfers averaged at 24.50 per day for four day for a total of 98.00. 21 Lodging/meals/Per Diem for staff attending the conference is averaged at 270.00 for four days for a total of 1,080.00. Contractual Consultant Costs ($0)—No funds are requested for this line item. Contract Trainers/Speakers ($3,000.00) —Funds are requested for one speaker or trainer for a special event for Kick Butts Day or the Great American Smoke-out including travel, lodging, meals, speaker fee, and other costs associated with the speaking engagement. Data Collection ($500.00)—The funds requested will provide for data collection by Cindy Kronague (Prime Solutions) or by a University of Northern Colorado Intern from the statistics department for one random survey of Johnstown/Milliken residents to determine support for secondhand smoke prevention/education and increasing the number of public places and workplaces that limit smoking.
