83rd Ave.just south of West 10th St., Greeley, CO and is funded with a Federal Bridge Replacement Grant. Sear-Brown
was selected as the consultant for the project to provide engineering design and construction management services.

o The Consultant's company name "Sear-Brown" shall be changed to "Stantec Inc.". The company Sear-
Brown was purchased by Stantec Inc. during the course of the contract. (See attached Exhibit A)
o Additional services to the original Scope of Work were required. Additional services included: (See attached
Exhibit B)
• Resetting section corner in State Hwy 34
• Supplementary surveying for landowner's utilities and driveway
• Design landowner's leach field impacted by the project
• Design landowner's private waterline impacted by the project
• Change hourly rate of pay for Stantec's construction staff

Dear Mr. Howard:
Thank you for allowing us to submit the following fee as additional services the Sheep Draw
Bridge Project (Bridge 27/58A). The fees below cover additional services rendered during
original design, and increases related to an updated Specific Rate of Pay table to be utilized for
final design and construction administration.
Additional Services during original Desikn
• Addition survey that was needed at the Morgan's property to perform a design of his
septic tank and a driveway entering the rear of his house $1,598
• Design adjustments to the Morgan leach field were required in order to remove a leach
line that was within the proposed right-of-way and replace a leach line outside of the
proposed right-of-way. Charge includes work performed by Stantec, Terracon and John
Brunner & Company. $3,545
• Additional design effort was required to assemble a set of plans for the construction of a
waterline service to the Morgan property. This was needed in order to allow Mr. Morgan
to install a new waterline prior to the start of the bridge project. $980

• Weld County requested that Stantec set a survey corner within the intersection of US-34
and WCR 27. $1,525
Additional Fee for Final Design and Construction Administration
• Weld County has agreed to compensate Stantec for final design and construction
administration based on an updated rate of pay. This is due to normal salary increases
since the signing of the original contract in August 2001 $903
• Additional mileage will be required in order to allow Stantec to visit the construction site
during construction. This is due to the fact that Bridge 27/58A and 13/27A were not
constructed simutaniously $481

Please submit the following for the Board's next agenda:
Change Order No. 3 for Weld County Bridge 27/58A over Sheep Draw.
Weld County has contracted with Stantec for design and construction inspection services
for the bridge replacement project for the amount of $169,718.00. Change Order No. 1
increased the contract amount by $7,456.67. The change order included additional
environmental clearance issues not anticipated in the original scope of work. Change
Order No. 2 was approved and allowed Stantec to make changes in staff working on the
project. This change order did not change the contract amount.
Attached is Change Order No. 3 and support documentation. This change order will
officially change the name on the original contract from Sear-Brown to Stantec, Inc. As
stated in Exhibit A, Sear-Brown has changed their name to Stantec during the course of
this contract. Also, this change order will increase the contract amount by $9,032.00 for
additional services that were outside the original scope of work. The additional services
include the following:
• Resetting section corner in State Hwy 34
• Supplementary surveying for landowner's utilities and driveway
• Design landowner's leach field impacted by the project
• Design landowner's private waterline impacted by the project
• Change hourly rate of pay for Stantec's construction staff
The new contract amount will be$186,206.67. The project is funded with a Federal Bridge
Replacement Grant and the current project cost and estimates, including this change order,
are within the project budget. The project is scheduled to be advertised for construction
early October, 2004 with completion in spring of 2005. -
