RE: APPROVE APPLICATION FOR FEDERAL ASSISTANCE FOR ENVIRONMENTAL
PROTECTION AGENCY GRANT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Application for Federal Assistance
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment,and the U.S. Environmental Protection Agency, commencing October 1, 2004,and
ending September 31, 2005,with further terms and conditions being as stated in said application,
and
WHEREAS,after review,the Board deems it advisable to approve said application, a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado,that the Application for Federal Assistance between the County of Weld, State
of Colorado, by and through the Board of County Commissioners of Weld County,on behalf of the
Weld County Department of Public Health and Environment,and the U.S. Environmental Protection
Agency be, and hereby is, approved.

Enclosed for Board review and signature is the Environmental Protection Agency(EPA) Grant.
The total amount requested is $13,072 for the funding period of October 1, 2004 through
September 31, 2005.
Weld County Department of Public Health and Environment will use this funding to supplement
programs already being initiated by the STEPS to a Weld County Health Educator. The funding
would also support the Wee Wheezers program which is an asthma education program for
parents of young children; it is an asthma education program in English and Spanish, based on
social learning theory, designed specifically for parents of young children under the age of seven.
The program provides parents with the knowledge, skills and motivation necessary to recognize,
prevent, and/or appropriately manage asthma symptoms; to use medical, educational and
interpersonal resources for information and support; communicate effectively with other adult
caregivers and physicians; and promote general well-being of the family unit.

Narrative & Anticipated Results
Asthma is categorized as a chronic disease,potentially fatal caused by inflamed airways that
restrict airflow. During the past two decades, the number of asthma cases and asthma related
deaths has increased—predominantly among the economically disadvantaged, urban, and
minority groups. Managing asthma requires long-term,multi-faceted approach, including the
use of patient education, behavioral changes, drug therapy, and avoidance of asthma triggers
such as irritants, viruses, and inhaled allergens to which a patient is sensitive. The National
Asthma Education and Prevention Program(NAEPP) has issued guidelines which emphasize the
importance of educating patients. The Asthma and Allergy Foundation reported that an asthma
patient experiences an average of 11.7 days of restricted activity yearly and this restricted
activity is the single largest reason that children miss school. Additionally, lost wok time costs
for caregivers of asthmatics and housekeepers also increased by 88%. CDC reported a 74%
increase in the prevalence of asthma in the U.S. among children and the number of deaths related
to asthma in children has nearly tripled over the past three decades. Compounding this disease is
the fact that Hispanics living with asthma report more asthma attacks and visits to emergency
departments in addition to visiting the doctor more frequently for urgent asthma care than other
ethnic groups.
One major avenue to combating the increased usage of the emergency department and increased
usage of physicians is to promote patient education. The Weld County Department of Public
Health &Environment(WCDPHE) intends on increasing patient awareness in children with

asthma through an Asthma and Allergy Foundation of America sponsored program called Wee
Wheezers. Wee Wheezers is an asthma education program for parents of young children; it is an
asthma education program in English and Spanish, based on social learning theory, designed
specifically for parents of young children under the age of seven. The program provides parents
with the knowledge, skills and motivation necessary to recognize, prevent, and/or appropriately
manage asthma symptoms; to use medical, educational and interpersonal resources for
information and support; communicate effectively with other adult caregivers and physicians;
and promote general well-being of the family unit. The program consists of four small group
sessions that last approximately two hours each. Children attend two sessions designed for them
along with their parents. Health professionals familiar with asthma management facilitate the
sessions. The results of the original research validating the effectiveness of this program indicate
improvement of five outcome measures with statistically significant improvement in symptom
free days and sleep interruption. There were also significant gains in parent knowledge about
asthma and asthma management. Each instructor packet includes:
• Instructor's Manual
• In-class videotape: Wee Wheezers Instructor's Video
• A parent videotape designed for at-home viewing—Wee Wheezers Companion Video
• A set of visual aids for instructors
• A set of reproducible handouts for parents
• A set of reproducible handouts for children
• A compact disc containing handouts, visual aids, and evaluation form
The lessons emphasize the parents' and children's responsibility for recognizing the early signs
and symptoms of asthma and for initiating the appropriate steps to care for asthma, including
trigger avoidance, adherence to medications, and medical help when necessary. Each of the first
three sessions has homework assignments for both the parents and the children; homework for
the children includes practicing belly breathing and completing an asthma diary with their
parents assistance.
Currently, the greater Greeley area houses thirty-eight(38) licensed preschool centers. The
WCDPHE will promote Wee Wheezers in twenty(20) of the thirty-eight preschools. The
estimated number of asthmatic children expected to benefit will be at least thirty(30). The Wee
Wheezers educator will teach the course in twenty preschools and the educator will also promote
the usage of the North Colorado Medical Center asthma management program. These two
programs will augment the Open Airways program which will be implemented in four(4)
Greeley elementary schools.
One of the goals of the EPA grant is to prevent Weld County residents from being hospitalized
for asthma through Weld County parents and children who currently have asthma with necessary
information to better manage asthma. The educational material will be provided through the
Wee Wheezers program and through information provided about current support systems within
the community. Additional program goals will aim towards giving parents the knowledge, skills,
and motivation to:
• Prevent asthma symptoms
• Appropriately manage symptoms
• Use medical, educational, and interpersonal resources appropriately for asthma care

• Communicate effectively with all adults responsible for the child's care
• Promote the psychosocial well-being of the family unit
Applicant Description
The internal infrastructure of the Weld County Department of Public Health&Environment
(WCDPHE),which was established in 1938, provides the ideal location for the community
intervention programs. The WCDPHE, both currently and historically, has strong financial
systems, experience administering grant programs, and can effectively distribute, manage and
monitor the funds and activities necessary to implement the Environmental Protection Agency
grant. The WCDPHE currently administers funds in excess of$5,400,000.
Weld County is a primarily agricultural county, covering 3,999 square miles in North Central
Colorado; it is bordered on the north by Wyoming and Nebraska and on the south by the Denver-
metropolitan area. The economy in Weld County is driven by agriculture. Fertile fields of green
can be found in every part of Weld County, which ranks as the third leading agricultural area in
the United States. Weld County is Colorado's leading producer of cattle, grain and sugar beets. It
is dominated by farming, ranching and small businesses. The third largest county in Colorado,
Weld County has an area greater than that of Rhode Island, Delaware and the District of
Columbia combined. There are 31 incorporated towns in Weld County, with Greeley acting as
the county seat and principal city. Between 2000-2002 Weld County was the second fastest
growing area in the state with a 13.4% increase in population. The greater Greeley area has a
population of 94,597 and is estimated to reach 122,404 by 2007. Weld County is 70% Anglo
and 30% Latino/Hispanic, reflecting an 84% increase in the Latino/Hispanic population since
1990. According to the Behavioral Risk Factors Surveillance System Data for Weld County,
30.3% of Latino's have no health insurance compared to 10% of Anglos living in Weld County.
The high rates of uninsured Latino residents may reflect the fact that a substantial percentage of
Latino workers are employed in low-wage jobs that provide annual incomes less than $30,000
and do not offer health insurance benefits. In addition, the Latino/Hispanic population primarily
services the laborer and farming occupations which average an uninsured rate of 32%.
Comparatively, the Behavioral Risk Factors Surveillance System Data for Weld County also
identified that 37.8% of the Latino/Hispanic population has family incomes below 200%poverty
compared to 22.7% of the Anglo population; again laborer's and farmer's had an average
percentage of 27% having family incomes below 200%poverty.
Staffing & Funding Sources
The WCDPHE is currently working through the STEPS to a Healthier Colorado grant, provided
by the Center for Disease Control to implement community based initiatives to promote better
health and prevent disease under the Health and Human Service's new STEPS to a HealthierUS
program. The grant supports innovative and exciting programs across the country that will
reduce the burden of diabetes, obesity and asthma on individual families, local communities and
the country as a whole. In September 2003, the Weld County Department of Public Health &
Environment was awarded $270,000 to apply staff to two essential public health services:
informing and educating the public about health issues and mobilizing partnerships in the
community to address chronic diseases. One of the major objectives within the first year of the
five year initiative is to increase the number of youth and adults with asthma who receive
education and improved health care. This objective will continue into the second year of funding

and will focus on the Latino/Latina youth. The STEPS project staff will rely upon relationships
which have already been established within the schools and continue to work with the population
to provide asthma education. The STEPS grant has a 1.0 FTE dedicated as the Asthma/Tobacco
Coordinator who will be working towards the Healthy People 2010 objectives for the next five
years. The Indoor Environments Unit, Air and Radiation Program, Office of Partnerships and
Regulatory Assistance funding will be used in conjunction with the STEPS grant to attain the
goal of providing asthma education and outreach projects that address environmental triggers for
asthma while promoting existing community outreach programs and the education of asthma
services.

**Preschools with higher Hispanic enrollment will be targeted when identifying and recruiting
preschools.
Evaluation Criteria
The Wee Wheezers curriculum contains a program evaluation section with forms to be
completed by the instructors and course participants. The results will be used to document and
assess the need for course improvements. The instructor evaluates Wee Wheezers by a pre-test
administered just prior to the education session and a post-test administered immediately after
the program. The evaluation instrument includes 20 multiple choice and true/false questions that
address asthma physiology, symptoms, triggers, medications, and delivery and measurement
devices. Results from other organizations who have implemented the program show an average
of about 30% increase in knowledge following the completion of the class.
A pediatric Quality of Life Assessment(Juniper) questionnaire gauges how much children are
bothered by asthma when doing certain activities and how well they manage their asthma before

receiving this education. The questionnaire is mailed to class participants six (6) months after
attendance with the assumption that, with appropriate therapy and self management skills learned
in the education sessions, quality of life will have improved. The results of the Juniper
questionnaire have indicated in the past a 22% improvement in quality of life.
