Project Title: Decreasing Diabetes Disparities in Weld County Health disparity issue or need to be addressed: There is a lack of available and affordable culturally appropriate diabetes self-management education and diabetes prevention information for low-income and/or Hispanic residents of Weld County. According to the 2004(United Way's) Compass Needs Assessment, over 50% of respondents in all areas of Weld County said lack of affordable medical care was a moderate or major problem in Weld County. Additionally, 69.2% of people with incomes < $14,999 and 60% of those with incomes between $15,000-$34,999 reported not having enough money to pay for medical needs (and was a moderate to major issue). Target population: Weld County's population is comprised of 30.81% Hispanic/Latino (EDAP, 2005). The rate of diabetes in Weld is 3.8% (BRFSS, 2004). The focus of this grant will be low-income, uninsured, and under-insured, monolingual Spanish speakers and older low-income residents who have diabetes or an increased risk of developing diabetes. Goal(s) and Objective(s): Goal 1) To establish a central point of contact to enhance the integration of prevention and education services related to the systematic care and treatment of patients with diabetes for the medically underserved and Hispanic populations in the community. This will be accomplished through outreach, educational classes, media enhancement, worksites and school wellness programs and ongoing communication with health care providers who care for the target population (through the North Colorado Health Care Alliance). 1. To hire bi-lingual/bi-cultural staff to implement program goals 2. To create system to do chart reviews of people with diabetes in the Electronic Medical Records (EMR) 3. Coordinate 3 trainings for health care professtionals Goal 2) To promote diabetes prevention in Weld County by establishing and supporting community education programs for people who have risk factors for developing diabetes. These programs will also improve self-management of diabetes for the medically underserved though regular education and support related to prevention of the devastating complications associated with this disease. 1. Provide regular Dining with Diabetes classes in the community at least four in English and four in Spanish each year, focusing on foods and traditions that are relevant to the target population 2. Provide at least four platicas (small discussion groups) a month using the Small Changes Make a Big Difference in homes or other locations focusing on low-income women who are responsible for young children. 3. Promote awareness of the importance of regular exercise by organizing and coordinating a neighborhood walking program for people who have attended a platica or a Dining with Diabetes class, with at least 20 regular participants. 4. Provide quarterly Diabetes Self-Management Classes in Spanish 5.Coordinate 5 a Day Food coupons for 500 WIC and Weld Food Bank participants to use to purchase fruits and vegetables at the Greeley Farmers' Market during the months of July, August, & September, 2006 6. To offer regular risk assessments and screenings in geographically convenient locations for the target population to identify those people who have elevated blood glucose, to help the target population understand their risk of developing diabetes, as well as providing education about how diabetes can be delayed or prevented How project will be accomplished: A project supervisor, preferably a bilingual Certified Diabetes Educator, will be hired to expand the work of the Weld County Diabetes Coalition. This includes hiring and training community outreach specialists (promotoras) to coordinate presentations in homes (platicas) and other locations, keeping updated on current prevention and treatment modalities to be able to serve as a resource for health care providers, and implement a variety of programs through a planned, systematic approach with cooperating agencies in the community. It will be expected that this supervisor will develop and maintain a high quality relationship with North Colorado Medical Center(local hospital), so that resources can be leveraged and duplication can be minimized/eliminated. In addition to the programs for the people diagnosed with diabetes, training for health care providers will be enhanced related to the Electronic Medical System, so that all Health Alliance patients with diabetes can be "flagged" for recommended screenings and tests, such a foot exams, eye exams, HgAlc tests, etc. Recommended follow-up can then be accomplished in a more integrated manner. Because the Weld County Department of Public Health and Environment, Sunrise Community Health Center, and Monfort Children's Clinic are all linked by the same Electronic Medical Record system, the project supervisor will be able to manage the care of the patients diagnosed with diabetes by adding "real time prompts" to patient records, create reports, and establish treatment protocols. Describe the interventions or approach selected and the desired outcomes The intent of the grant will be to leverage limited resources in this community related to the prevention and treatment of diabetes through (1) the development of a systematic and integrated approach to diabetes in Weld County; to 2) meet the target population where they're at: helping them to understand the risk factors associated with the development of diabetes, and providing them with information that will help current diabetics prevent life-altering and costly complication by motivating them to change their behavior related to activity and nutrition, and 3) fund a variety of collaborative efforts in the community to increase awareness, support and education. The Project Coordinator will be expected to work with members of the Weld County Diabetes Coalition to offer regular evidence-based interventions to both mono-lingual and bilingual diabetics, as well as other low-income Weld County residents who have been determined to be at risk for diabetes. The long term outcome is centered on chronic disease prevention, specifically diabetes, in the underserved targeted population. These goals are ethically, economically, and culturally aligned to improve the quality of life for the target population. Why this approach was chosen: This approach is the result of months of discussion by the Weld County Diabetes Coalition about what the needs are in this community related to the prevention and treatment of diabetes. Total amount of funds requested: $222.611.00 What are plans for sustainability after the grant period ends? It is hoped that once the infrastructure of the diabetes services is created, the partners will be able to continue to provide the improved and enhanced medical care. Many of the pieces are already being done, and with a more cohesive plan, the services will continue even after the funding is no longer available. The project coordinator would have the responsibilities of managing the grant, arranging the contracts with CSU Extension and the diabetes educator, implementing the chart review system in the Electronic Medical Record System, organizing the health care provider trainings, and coordinating the Farmer's Market Food Coupons program. The bi-lingual/bicultural outreach specialist(s) would have the primary responsibilities for the health education in the home (platicas), the Dining with Diabetes classes in Spanish in the schools, coordinating the walking program, and referrals to the diabetes self-management classes. Travel Expenses: The county rate for mileage in 2006 will be 44.5 cents a mile. This is about 500 miles a month per person. The registration is to take advantage of an opportunity to attend The Physical Activity and Nutrition National Conference which is focusing on obesity and diabetes prevention being held in Denver in May, 2006. Itemized Contractual and Sub-Contracts: The CSU Extension agent is experienced at presenting the Dining With Diabetes (DWD) curriculum, but it is not included in her "scope of work." The grant funding would "buy out" her time to do six series of four classes, plus all the preparation time and materials, one each quarter. Funding is also included to contract with a bilingual Certified Diabetes Educator (CDE) or Nurse Practitioner (NP) to teach the diabetes self-management classes. The Health Department currently uses grant funds for a half-hour Health Talk show on Saturdays at 1 p.m. on the Spanish language radio station. This grant includes funding to continue that, as well as pay bilingual diabetes educators to do several programs each year. Radio is a good way to do health education in Spanish. Itemized Supplies: This category includes the gift cards used as incentives for the hostesses in the health education in the home (platicas) to invite their family and friends and provide healthy snacks. Also included are the pedometers for the walking program, notebooks and materials for Dining with Diabetes (25 persons per class), coupons for WIC and Weld Food Bank clients to use at the Greeley Farmers' Market, printing and other expenses. Itemized Equipment Expenses: This includes two computers, as well as a hemoglobin Alc analyzer and test cartridges. This machine will be used primarily on the mobile medical van with their migrant outreach program. It can also be used at various educational events and at Diabetes Sundays with the American Diabetic Association. Project Title: Decreasing Diabetes Disparities in Weld County Measures or indicators to be used. Attendees will be counted at each of the presentations, such as the plkticas, the Dining with Diabetes classes, the trainings for Health Care Providers, walking programs, etc. Also the number of patient charts that are flagged with real time prompts, and then the number of specific issues that the health care providers address during office visits can be identified on reports from the Electronic Medical Records (EMR). Methods/strategies used to determine effectiveness and impact on health disparities. Weld County is currently being oversampled on the Behavior Risk Factor Surveillance Survey, so the number of people diagnosed with diabetes who report having received diabetes education should increase. In the long term, there might also be reduced rates of diabetes, and less amputation, less blindness, and less kidney dialysis as a result of delayed complications from diabetes. How will results be used, disseminated and communicated. Articles in the Greeley Tribune, and the Spanish weekly newspaper, La Tribuna, can help both create interest in attending a class, such as Dining with Diabetes, and help inform the community about the success of the programs. There is an increasing incidence of diabetes across the US and around the world. This trend is alarming to Weld County public health professionals, especially because 30%of Weld County residents are Hispanic, with an increased risk of developing diabetes. The Greeley Evans School District has partnered with WCDPHE for the past two years to promote healthy eating and physical activity. With support from the Steps to a Healthier Weld County grant and WCDPHE, we have been able to increase educational opportunities about healthy lifestyles to our students, parents, and staff and establish six Coordinated School Health teams in our district. We would like to expand on our current efforts by offering"Dining with Diabetes"classes to our Spanish speaking parent population, as well as other diabetes prevention materials/programs. This information is particularly valuable for the uninsured and underinsured population that our district serves. In the past, we have found that school sites are appropriate interventions sites because they are easily accessible to our families, and they are a trusted institution. The Greeley Evans School District has been an active partner in addressing other health issues in the community. We know that the health of a community depends on agencies working together to plan and implement programs.
