RE: APPROVE TEN PURCHASE OF SERVICES AGREEMENTS FOR OUTREACH
ACTIVITIES AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with ten Purchase of Services Agreements for
Outreach Activities between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Human Services, Area
Agency on Agency, and the various Senior Centers, as follows, commencing July 1, 2005, and
ending June 30, 2006, with further terms and conditions being as stated in said agreements, and
Tri-Town Senior Center- $500 Hill N Park Senior Center- $500
Erie Senior Center - $500 Johnstown Senior Center - $500
Evans Senior Center - $500 Kersey Senior Center- $500
Fort Lupton Senior Center- $500 Mead Senior Center - $500
Greeley Senior Center- $500 Pierce Senior Center - $500
WHEREAS,after review,the Board deems it advisable to approve said agreements, copies
of which are attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the ten Purchase of Services Agreements for Outreach Activities between
the County of Weld,State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Services,Area Agency on Agency,and the various
Senior Centers as listed above be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreements.

SUBJECT: Purchase of Services agreement between the Weld
County Area Agency on Aging and Weld County Senior
Centers
Enclosed for Board approval are ten (10) Purchase of Service agreements with Weld County
Senior Centers to purchase outreach activities. These grants are in response to needs data which
shows that a high percentage of Weld seniors are lonely and isolated from community life. The
senior centers listed below have agreed to redouble their efforts to reach out to these seniors by
implementing telephone reassurance calls, one on one visitations, and buddy programs to attract
new seniors to the centers and a variety of other outreach activities. The effective date for the
grants is July 1, 2005 through June 31, 2006. Grants will be given to the following:
Tri-Town- $500
Erie Senior Center- $500
Evans Senior Center- $500
Fort Lupton Senior Center- $500
Greeley Senior Center-$500
Hill N Park Senior Center - $500
Johnstown Senior Center- $500
Kersey Senior Center-$500
Mead Senior Center-$500
Pierce Senior Center-$500

Purchase of Services Agreement
THIS AGREEMENT, made and entered into this Pt day of July, 2005 by and between the County of Weld, State of
Colorado,by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division
of Human Services'Area Agency on Aging, hereinafter referred to as "AAA" and the Tri-Town Senior Center hereinafter
referred to as "Tri-Town."
WITNESSETH:
WHEREAS, "AAA" develops and administers the Area Plan for a comprehensive and coordinated system of services,
and
WHEREAS, "AAA"considers senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Tri-Town" is a senior center providing recreational, nutritional, and educational opportunities for senior
citizens; and
WHEREAS, "Tri-Town"proposes to implement a"The Tri-Town Loneliness"program to reach out to area seniors;
NOW THEREFORE, it is mutually agreed:
1. The "AAA"hereby agrees to award grant monies to "Tri-Town" in the sum of$500.00.
2. "Tri-Town"agrees to maintain a volunteer committee that will reach out to homebound and isolated seniors and
encourage senior center involvement and/or provide home visits to homebound seniors. Volunteers will also send
cards, letter,brochures and flyers to community seniors.
3. "Tri-Town"agrees to document contacts made. "Tri-Town"understands that the $500.00 may be used to pay for
the cost of the outreach program and to advance the Tri-Town Senior Program.
4. "Tri-Town"understands this is a one time only award and the terms of this agreement are valid through June 30,
2006.

referred to as "Erie."
WITNESSETH:
WHEREAS, "AAA"develops and administers the Area Plan for a comprehensive and coordinated system of services,
and
WHEREAS, "AAA"considers senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Erie"is a senior program providing recreational,nutritional, and educational opportunities for senior
citizens; and
WHEREAS, "Erie"proposes to implement a"Connect to the Community"program to reach out to seniors in their
community to involve them in the social activities at the Erie Senior Program;
NOW THEREFORE, it is mutually agreed:
1. The "AAA"hereby agrees to award grant monies to the "Erie"in the sum of$500.00.
2. "Erie"agrees to maintain the outreach program focused on isolated seniors using volunteers from the senior center
and other community entities. "Erie"will be developing events that will focus on multiple cultures i.e. Martin
Luther King Day, Cinco De Mayo and St. Patrick Day hoping to encourage homebound and isolated seniors into
becoming active in the senior center and the congregate meal program.
3. "Erie" agrees to document contacts made. Erie Senior Citizens understand that the grant of$500.00 may be used to
implement the"Connect to the Community" and to advance the overall Erie senior recreational program.
4. "Erie"understands this is a one time only award and the terms of this agreement are valid through June 30, 2006.

referred to as "Evans."
WITNESSETH:
WHEREAS, "AAA" develops and administers the Area Plan for a comprehensive and coordinated system of services,
and
WHEREAS, "AAA" considers senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Evans"is a senior center providing recreational, nutritional, and educational opportunities for senior
citizens; and
WHEREAS, "Evans"proposes to implement a"Reach Out and Touch Someone"to reach out to area seniors;
NOW THEREFORE, it is mutually agreed:
1. The "AAA"hereby agrees to award grant monies to "Evans" in the sum of$500.00
2. "Evans" agrees to maintain an outreach volunteer committee that will coordinate all senior outreach efforts. The
volunteers will make contact via phone and/or face to face to encourage participation at the senior center. Seniors
will receive information concerning ongoing and special events through a variety of mailing and advertizing.
3. "Evans" agrees to document the results of this outreach effort on a monthly bases. "Evans"understands that the
grant of$500.00 may be used to pay for the cost of the outreach program.
4. "Evans"understands this is a one time only award and the terms of this agreement are valid through June 30, 2006.

hereinafter referred to as "Fort Lupton."
WITNESSETH:

WHEREAS, "AAA"considers senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Fort Lupton" is a senior center providing recreational,nutritional, and educational opportunities for senior
citizens; and
WHEREAS, "Fort Lupton"proposes to implement a"Outreach to Isolated Elderly"program in order to connect with
area seniors;
NOW THEREFORE, it is mutually agreed:
1. The "AAA"hereby agrees to award grant monies to "Fort Lupton"in the sum of$500.00.
2. "Fort Lupton"agrees to continue to maintain the"Outreach to Isolated Elderly"program . The program will
continue to focus on the connecting with community seniors through special events, programs and scholarships for
low income and isolated seniors.Transportation will be provided to seniors who can not access events on their own.
3. "Ft. Lupton"agrees to document contacts made. "Ft.Lupton" understand that the want of$500.00 may be used to
implement the"Operation Connect" and to advance the overall senior recreational program.
4. "Fort Lupton"understands this is a one time only award and the terms of the agreement are valid through June 30,
2006.

hereinafter referred to as "Greeley"
WITNESSETH:
WHEREAS, "AAA"develops and administers the Area Plan for a comprehensive and coordinated system of services,
and
WHEREAS, "AAA"considers senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Greeley"is a senior center providing recreational,nutritional, and educational opportunities for senior
citizens; and
WHEREAS, "Greeley"proposes to implement a"Shape Up"program to reach out to area seniors;
NOW THEREFORE, it is mutually agreed:
1. The"AAA"hereby agrees to award grant monies to "Greeley"in the sum of$500.00.
2. "Greeley" agrees to send flyers to low income isolated seniors inviting them to apply for an Exercise Scholarship
that would cover the cost or a portion of the cost for the Cardiovascular and Weight Training class. This class
promotes physical fitness and positive social interaction. Transportation will be provided by the volunteers for
seniors who can not provide or obtain access to transportation services.
3. "Greeley"understands this is a one time only award and the terms of the agreement are valid through June 30, 2006.

WHEREAS,"Hill N Park"proposes to implement a loneliness outreach program to reach out to seniors in the Hill N Park area to
involve them in the social activities at the Hill N Park Senior Center;
NOW THEREFORE, it is mutually agreed:
1. The"AAA"hereby agrees to award grant monies to"Hill N Park"in the sum of$500.00.
2. "Hill N Park"agrees to implement an outreach project that would involve contacting seniors in the Hill N Park community and
encouraging involvement at the center. Phone outreach,information flyers, guest meal passes will be a few of the techniques
used to connect with isolated community seniors.
3. "Hill N Park"agrees to document the results of this outreach effort on a monthly bases."Hill N Park"understands that the grant
of$500.00 may be used to pay for the cost of the outreach program and to advance the overall"Hill N Park"senior citizens
program.
4. "Hill N Park"understands this is a one time only award and the terms of this agreement are valid through June 30,2006.

and
WHEREAS, "Johnstown"is a program providing recreational, nutritional, and educational
opportunities for senior citizens, and
WHEREAS, "Johnstown"proposes to implement a"Johnstown Senior Outreach."to reach out to
seniors in the Johnstown area to involve them in the social activities at the Johnstown
Community Center;
NOW THEREFORE, it is mutually agreed:
1. The"AAA"hereby agrees to award grant monies to "Johnstown"in the sum of$500.00.
2. "Johnstown" agrees to implement "Johnstown Senior Outreach", an outreach project that
would involve the maintenance of a volunteer committee that will continue to reach out to
isolated or homebound seniors via monthly mailing of flyers/calendars/newsletters and
phone and one on one outreach. Special programs such as ice cream socials and other social
outreach events will be implemented and/or maintained. Local media options will be used
to advertise events and provide service information.
3. "Johnstown" agrees to document the results of this outreach effort on a monthly bases.
"Johnstown" understands that the grant of$500.00 may be used to pay for the cost of the
outreach program and to advance the overall "Johnstown" senior program.
4. "Johnstown" understands this is a one time only award and the terms of this agreement are
valid through June 30, 2006.

WHEREAS, "AAA" considers senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Kersey" is a senior center providing recreational, nutritional, and educational opportunities for senior
citizens; and
WHEREAS, "Kersey"proposes to implement a"Reach Out"program to find homebound seniors in their community and
make a difference in their lives;
NOW THEREFORE, it is mutually agreed:
1. The"AAA"hereby agrees to award grant monies to "Kersey"in the sum of 500.00.
2. "Kersey" agrees to maintain a volunteer outreach committee that will identify homebound and/or isolated seniors
and provide home visits that will include a welcome gift(or other items that are needed in the home) and a coupon
to attend the congregate meal program.
3. "Kersey"agrees to document the results of the program on a monthly bases. "Kersey"understands that the$500.00
may be used to pay for the cost of the outreach program and to advance the overall senior program.
4. "Kersey"understands this is a one time only award and the terms of the agreement are valid through June 30, 2006.

WHEREAS, "Mead"is a program providing recreational,nutritional, and educational opportunities for senior
citizens, and
WHEREAS, "Mead"proposes to implement a"Building Bridges of Friendship Program"to reach out to seniors in
the Mead and Carr areas to involve them in the social activities at the Mead Senior Center;
NOW THEREFORE, it is mutually agreed:
1, The"AAA"hereby agrees to award grant monies to the"Mead"in the sum of$500.00.
2. "Mead"agrees to recruit volunteers who will contact homebound seniors and new seniors to the area, to
increase phone calls and mailings, develop special events and to help seniors with transportation,when
possible.
3. "Mead"agrees to document activities and results of this outreach effort on a monthly. "Mead"understand that
the grant of$500.00 may be used to pay for the costs of the outreach program and to advance the overall Mead
Senior Citizens Program.
4. "Mead"understands this is a one time only incentive award and the terms of this agreement are valid through
June 30, 2006.

to involve them in the social activities at the Pierce Senior Center;
NOW THEREFORE, it is mutually agreed:
I. The "AAA"hereby agrees to award grant monies to "Pierce"in the sum of$500.00.
2. "Pierce"agrees to maintain a volunteer outreach committee who will contact homebound seniors and providing
home visits to homebound and isolated seniors,meal tickets, birthday cards, and other special projects. "Pierce"will
also reach out to new community members and encourage involvement at the center by providing a variety of
services such as access to the center's computer and other social events.
3. "Pierce"agrees to document activities and results of this outreach effort. "Pierce"understands that the grant of
$500.00 may be used to pay for the costs of the outreach program and to advance the overall Pierce Senior Center
Program.
4. "Pierce"understands this is a one time only award and the terms of this agreement are valid through June 30, 2006.
