HomeMy WebLinkAbout20051211.tiff RESOLUTION RE: APPROVE NORTH COLORADO MEDICAL CENTER FOUNDATION GRANT REQUEST AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS,the Board has been presented with a North Colorado Medical Center Foundation Grant Request between the County of Weld,State of Colorado,by and through the Board of County Commissioners of Weld County,on behalf of the Department of Human Services,Area Agency on Aging, with further terms and conditions being as stated in said application, WHEREAS,after review,the Board deems it advisable to approve said application,a copy of which is attached hereto and incorporated herein by reference. NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County,Colorado,that the North Colorado Medical Center Foundation Grant Request between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,on behalf of the Department of Human Services,Area Agency on Aging be,and hereby is, approved. The allotted funds will be used to enhance services through the Weld County Area Agency on Aging Dental, Vision, and Hearing Program. We are requesting $10,000. The Dental, Vision, and Hearing Program provides financial assistance for the eligible 60 and older population of Weld County who are in need of dental, vision, and/or hearing services. If you have additional questions, please contact Kathleen Alvarado at extension 3314. 2005-1211 North Colorado Medical Center Foundation Grant Request: May 3, 2005 General Information The Weld County Area Agency on Aging is a support system for the residents of Weld County who are 60 years of age and older as well as their caregivers. The Area Agency on Aging is a county agency in the Human Services Department, governed by the Weld County Board of Commissioners and receives support from a 16-member community board. The primary goal of the agency is to help older individuals maintain independence and dignity in their home and community. Support services for older adults are provided directly by the Area Agency on Aging or through contracts with community agencies. The Area Agency on Aging is also a help source for many older adults and may be contacted for a wide range of information and referral questions. Some of the services provided by the Area Agency on Aging include Legal Assistance, Senior Nutrition, Peer Counseling, Hispanic Elderly Outreach Program, Family Caregiver Support Program, Long Term Care Ombudsman, and Case Management. Description and Purpose of Program/Project In 1998, the Weld County Area Agency of Aging implemented a Dental, Vision and Hearing Program. Funding for this program was received from a special allocation from the State of Colorado to local Area Agencies on Aging. This particular funding was set aside to assist older adults in the community with services they were not able to receive as a result of financial deficits. The Area Agency on Aging saw the value for a dental, vision and hearing program because of the tremendous need and benefits it would provide the older adults in our community. The purpose of the program would be to help low income Weld County residents age 60 and older who are in need of monetary assistance for dental, vision, and hearing services and/or assistive devices such as glasses, dentures or hearing aids. As the mission of the NCMC Foundation is to enhance the health of the community and its citizens, the Area Agency on Aging also feels that our Dental, Vision and Hearing Program will follow suit with that statement. By providing these services to needy older adults of Weld County, it will have a great effect on the individual's health, quality of life and overall well-being. Supportive Research Complications with dental, vision and hearing issues could reduce the quality of life of an older adult. According to the Centers for Disease Control and Prevention in 2001, dimming eyesight and failing hearing can reduce physical, functional, emotional and social well-being (Desai et. al, 2001). Having both hearing and vision impairments affected 1,724,000 older adults age 70 and older. Those with both hearing and vision impairments were two times more likely than peers without impairments to report difficulty walking as well as have fallen during the preceding 12 months (Campbell et. al, 1999). These falls can lead to broken hips and other health problems, which can make everyday life more difficult. Older adults also deal with dental problems. Approximately 30 percent of those 65 and older have no natural teeth. Having missing teeth can affect areas such as nutrition where those missing teeth prefer to eat soft, easily chewed foods avoiding necessary foods such as fruits and vegetables (CDC, 2003). Based on data gathered from a strength and needs assessment of older adults in 2000, 10 percent of the 22,368 people 60 and older in Weld County needed either dental, vision or hearing assistance, which they were not able to afford (NRC Inc, 2004). Currently, the Dental, Vision and Hearing Program has over 50 individuals on the waitlist for the 2005-2006 fiscal year which begins in July. There is a great need for assistance and not enough finances to assist these needy older adults. The Weld County Area Agency on Aging was aware of the tremendous impact the program would have on the lives of older adults in the community. Client M.L. wrote a comment stating, "I'm pleased with the help I got for my hearing. Living on a fixed income, this has helped me enjoy hearing again. Thank you so much!" Client S.K. eagerly wrote, "Words cannot express how help from your program has helped my quality of life. People have told me they no longer need to shout and repeat conversations again and again to me, because I couldn't hear what was being said." One of our community providers, Michelle Gross, an audiologist in Ft. Lupton stated, "For those people I have provided services for, without the funds available from the Agency on Aging, hearing aids would not have been possible." Since the induction of the program, the agency has seen an improvement in the quality of life that the Dental, Vision and Hearing program has brought to older adults in our community. Population Served The funding for this grant will be used to target low income Weld County residents age 60 and older. Being a local agency funded by the Older Americans Act, those individuals we serve must be at least 60 years old. We are targeting those individuals who are in need of financial assistance for dental, vision or hearing services. The services that the Dental, Vision and Hearing Program provides is not limited to just one service. In the past, we have had many older adults ho received a combination of the three services to maintain their health and ensure their well-being. When the applicant has been accepted, they are tracked using SAMS 2000, the agency's data collecting system. This allows us to organize the information of the client, service and provider. With greater funds for the program, it would be able to provide the much needed services to a greater number of older adults throughout Weld County. Amount Requested The Area Agency on Aging is requesting $10,000 to further continue our Dental, Vision and Hearing program for the 2005-2006 fiscal year. The request for funding is due to the growing number of older adults in Weld County who are in need of dental, vision or hearing assistance. The older adult population of Weld County is expected to grow by 13,442 or 60% between 2000 and 2012 (NRC Inc, 2004). As a local agency, annual funding we receive from the State of Colorado has been decreasing due to the status of the State General Funds. We strongly believe that there is a need for these services, however we do not have the secure funds to actively pursue this growing need alone. With funding from the Northern Colorado Medical Center Foundation and other local foundations, we hope to continue this program to serve more individuals throughout Weld County. Committed Funding Sources The Weld County Area Agency on Aging receives funding for the Dental, Vision and Hearing program largely through state general funds. We have received $59,139.00 for the 2004-2005 fiscal year from state funding. The funding cycle for the program begins in July and ends in June of the following year. The Dental, Vision and Hearing Program also has received a grant, Community Health Investment Program (CHIP), from the Platte Valley Medical Center in Brighton, Colorado. The money from this grant is strictly to be used for southern Weld County, from Platteville to the southern part of the county. However, in the northern part of Weld County, there are over 50 individuals who have been waiting months, some close to a year for assistance. With the growing need and lack of funding, we are not able to meet the needs of these older adults. We continue to view the state of Colorado as our primary source for funding for the program. However, we are asking for assistance from the Northern Colorado Medical Center Foundation to provide the necessary funds to further continue and expand the Dental, Vision and Hearing Program in Weld County.
