RE: APPROVE ADDENDUM TO AGREEMENT FOR COUNTY CORONER SERVICES AND
AUTHORIZE CHAIR TO SIGN - COLORADO PATHOLOGY ASSOCIATES
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with an Addendum to the Agreement for County
Coroner Services between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, and Colorado Pathology Associates, commencing
January 1, 2006,and ending December 31,2008,with further terms and conditions being as stated
in said addendum, and
WHEREAS,after review,the Board deems it advisable to approve said addendum, a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado,that the Addendum to the Agreement for County Coroner Services between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, and Colorado Pathology Associates be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said addendum.
The above and foregoing Resolution was, on motion duly made and seconded,adopted by
the following vote on the 3rd day of October, A.D., 2005.

ADDENDUM TO THE AGREEMENT BETWEEN THE BOARD OF WELD
COUNTY COMMISSIONERS AND COLORADO PATHOLOGY FOR COUNTY
CORONER SERVICES
This addendum shall serve as written consent by both parties per Section III-Term of the
agreement between the Board of Weld County Commissioners, and Colorado Pathology
Associates to extend the agreement for County Coroner Services for an additional three-
year term from January 1, 2006 to December 31, 2008.
The blanket rate per autopsy shall be $900.00 effective January 1, 2006. Colorado
Pathology Associates also has the right to bill the District Attorney and Public Defender
for consultative time, travel, and testimony in accordance with rates specified by the State
of Colorado.
This addendum is signed this 3rd day of October, 2005.
