RE: APPROVE COLORADO DEPARTMENT OF TRANSPORTATION PERMISSION TO
ENTER PROPERTY FORM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Permission to Enter Property Form
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Public Works, to the Colorado
Department of Transportation with further terms and conditions being as stated in said form, and
WHEREAS,after review,the Board deems it advisable to approve said form,a copy of which
is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Permission to Enter Property Form from the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Public Works, to the Colorado Department of Transportation be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said form.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 21st day of March, A.D., 2005.

The undersigned property owner, owner's representative, or lessee hereby grants permission to the Colorado
Department of Transportation, its contractors, agents, and all others deemed necessary by the Department, to
enter the property identified above and perform the following activities:
DESCRIPTION OF WORK TO BE DONE
°Perform preliminary survey, including; recovery of and measurements to horizontal and vertical survey marks; topo-
graphic survey which includes the modeling of ground terrain and the measuring and mapping of physical features;
and the placement of temporary survey stakes or project control markers.
• Conduct preliminary environmental surveys to identify the presence of archaeological, historical, paleontological, or
ecological resources, and perform noise or air quality measurements. These surveys may include the exterior inspec-
tion of structures and buildings; the collection of chipped stone, biological, botanical, or fossil specimens; or sampling
of subsurface soil material.
• Perform real estate appraisal, or other physical inspections necessary to review the property, including any structures,
for possible acquisition.
This permission to enter does not include any drilling, digging, or sampling of materials beyond the use of hand tools. Any
holes dug will be backfilled to their original condition prior to leaving the site.

Dear Owner,
Western States Surveying, Inc. is working on a project along Interstate 25 for the
Colorado Department of Transportation and URS Corporation. Enclosed is a Colorado
Department of Transportation Permission to Enter Property form.
Western States Surveying, Inc. will be performing preliminary surveys, including
recovery of monuments and a topographic survey along the right of way and may need to
access your property where it abuts the highway right of way. Our survey may access
your property that lies north of State Highway 119 and south of State Highway 66. Weld
County Parcel 13 1302000070 and 131302000071.
Western States Surveying, Inc. is requesting that you grant us permission to perform this
work by completing and signing the enclosed form. We have contacted some owners by
phone and we thank you if you have given us permission,however the Colorado
Department of Transportation also requires us to get a signature on this form. Western
States Surveying, Inc. thanks you in advance for your time and cooperation. Please sign,
keep the yellow(canary) copy and return the white and pink copies.

WELD COUNTY PUBLIC WORKS HAZARD
AWARENESS/VISITOR SAFETY TRAINING
The following safety rules have been developed for your protection. Your cooperation as a visitor to our facility by conforming
to the following requirements will be appreciated. Failure to adhere to these requirements may result in you being denied
access to Weld County Public Works gravel pits.
Personal Protective Equipment
1. Wearing a hard hat is mandatory at all times when outside vehicles in the gravel pit areas and where
indicated by posted signs.
2. Safety glasses and ear protection is recommended around crusher or screen.
Traffic Control
1. Traffic flow is on the right. Always drive on the right side of the road and obey posted speed limits.
2. Obey the speed limit posting at each gravel it.
3. Look and listen for large trucks and other equipment. The operator's visibility is limited. Our large vehicles
have the right-of-way. Listen for backup alarms and ensure you are not in the path of the moving vehicle.
Ensure operators of large trucks and other equipment see you-get a positive acknowledge from them such as
a hand signal.
4. Do not drive close to or park next to high walls/slopes.
Equipment and Machinery
1. Before working on equipment, check with the supervisor in charge of the area you are assigned to work.
2. Mechanics and repair persons are required to wear steel toe footwear.
3. Passengers are not permitted to ride anywhere on a vehicle not intended for passengers.
4. Keep hands and feet away from moving parts.
5. Stay alert, always look for and obey warning signs.
6. Never dismount from a moving vehicle.
Your Conduct and Safety
1. Watch for Danger and Warning signs throughout property and never enter roped/chained off areas.
2. Truck drivers MUST remain in vehicle during loading material in the pit area.
3. Seat belts must be worn at all times when the vehicle is moving.
4. Tarping is to be done away from load area.
5. Don't step over, walk on, or walk under moving conveyers.
6. Use designated walkways/catwalks. Watch your step and use handrails. Watch for low overhead clearances.
7. Always walk-never run unless there is imminent danger.
8. Horseplay, fighting, and practical jokes are prohibited
9. The use of or being under the influence of alcohol or a controlled substance is strictly prohibited.
10. Bringing illegal drugs, alcohol, guns or other dangerous weapons on the property is prohibited.
Accidents/Emergencies
I. The emergency number is 911.
2. Report all emergencies, accidents, "close calls," and injuries promptly to the person that cleared your visit.
