WELD COUNTY, COLORADO

The Board of County Commissioners of Weld County, Colorado, met in regular session as the Board of
Social Services in full conformity with the laws of the State of Colorado at the regular place of meeting in
the Weld County Centennial Center, Greeley, Colorado, April 6, 2005, at the hour of 9:00 a.m.
ROLL CALL: The meeting was called to order by the Chair and on roll call the following members were
present, constituting a quorum of the members thereof:
Commissioner William H. Jerke, Chair
Commissioner M. J. Geile, Pro-Tem
Commissioner David E. Long - EXCUSED
Commissioner Robert D. Masden
Commissioner Glenn Vaad
Also present:
County Attorney, Bruce T. Barker
Acting Clerk to the Board, Carol A. Harding
Director of Finance and Administration, Don Warden
MINUTES: Commissioner Masden moved to approve the minutes of the Board of Social Services meeting
of April 4, 2005, as printed. Commissioner Geile seconded the motion, and it carried unanimously.
WARRANTS: Don Warden,Director of Finance and Administration, presented the warrants for approval
by the Board in the amount of $27,641.18. Commissioner Masden moved to approve the warrants as
presented by Mr. Warden. Commissioner Geile seconded the motion, which carried unanimously.
NEW BUSINESS:
CONSIDER AGREEMENT TO PURCHASE CHILD PLACEMENT AGENCY SERVICES AND AUTHORIZE
CHAIR TO SIGN-OPPORTUNITY IN LIVING:Judy Griego,Director of Social Services,stated Opportunity
In Living will be reimbursed at a rate based on the Needs Based Care Assessment levels that include room,
board,and treatment services or the Children's Habilitation Residential Program rate,and the term of the
agreement is January 27, 2005, through June 30, 2005. Commissioner Geile moved to approve said
agreement and authorize the Chair to sign. Seconded by Commissioner Vaad, the motion carried
unanimously.

CONSIDER AMENDMENT TO THREE NOTIFICATION OF FINANCIAL ASSISTANCE AWARDS FOR
FOSTER PARENT CONSULTATION PROGRAM WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR
TO SIGN: Ms. Griego stated this amendment will adjust the group rate from $350 per group of six foster
parents to a rate of$58.33 per participant. She stated the award period is June 1, 2004,through May 31,
2005. Responding to Chair Jerke, Ms. Griego stated this is in response to problems created for payment
when the entire group was not present. Commissioner Geile moved to approve said amendment and
authorize the Chair to sign. Seconded by Commissioner Vaad, the motion carried unanimously.
CONSIDER AMENDMENT OF CHILD PROTECTION AGREEMENT FOR SERVICES AND AUTHORIZE
CHAIR TO SIGN - ISLAND GROVE REGIONAL TREATMENT CENTER, INC.: Ms. Griego stated this
amendment will increase the funding for Island Grove Treatment Center from $120,000 to$195,000 for
treatment services and to include a fee schedule for Swab testing and confirmation services. Responding
to ChairJerke,Ms.Griego stated both the parents and teenagers are the clients in this program. She stated
the Court requires the department to show they are making all reasonable effort, including treatment, to
reunite the family. Commissioner Geile moved to approve said amendment and authorize the Chair to sign.
Seconded by Commissioner Masden, the motion carried unanimously.
CONSIDER TWO AGREEMENTS TO PURCHASE CHILD PLACEMENT AGENCY SERVICES FROM
VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN: Ms. Griego stated the vendors will be
reimbursed at a rate based on the Needs Based Care Assessment levels that include room, board, and
treatment services. Commissioner Geile moved to approve said agreements and authorize the Chair to
sign. Seconded by Commissioner Vaad, the motion carried unanimously.
Let the minutes reflect that the above and foregoing actions were attested to and respectfully submitted by
the Acting Clerk to the Board.
There being no further business, this meeting was adjourned at 9:20 a.m.
