RE: APPROVE AMENDMENT OF AGREEMENT TO PURCHASE RESIDENTIAL
TREATMENT CENTER SERVICES AND AUTHORIZE CHAIR TO SIGN - EL PUEBLO
BOYS AND GIRLS RANCH
WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with an Amendment of Agreement to Purchase
Residential Treatment Center Services between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County,on behalf of the Department of Social
Services, and El Pueblo Boys and Girls Ranch, commencing June 30, 2005, and ending
September 30, 2005, with further terms and conditions being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE,BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Amendment of Agreement to
Purchase Residential Treatment Center Services between the County of Weld,State of Colorado,
by and through the Board of County Commissioners of Weld County,on behalf of the Department
of Social Services, and El Pueblo Boys and Girls Ranch be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 12th day of October, A.D., 2005, nunc pro tunc June 30, 2005.

RE: Amendment of Agreement to Purchase Residential Treatment Center(RTC)
Services with El Pueblo Boys' and Girls' Range
Enclosed for Board approval is an Amendment of Agreement to Purchase RTC Services between
the Weld County Department of Social Services (Department)and El Pueblo Boys' and Girls'
Rnage. The Amendment was reviewed at the Board's Work Session held on June 27, 2005.
The Amendment provides for a 90 day extension to end September 30, 2005, with an update to
their current rates of$32.50 per day RTC and $57.30 per day in RCCF care. All other provisions
in the original Agreement to remain the same.

TH15 AMENDMENT made this'7—day of SO-,2005 between the Board of Weld County Commissioners,
sitting as the Board of Social Services,on behalf of the Weld County Department of Social Services,hereinafter
called "County"and El Pueblo Boys'and Girls'Ranch, One El Pueblo Ranch Way,Pueblo, CO 81006,hereinafter
called"Contractor".
WHEREAS,the Colorado State Department of Social Services,hereinafter called"State Department"is
authorized to provide social services to individuals and families of individuals through its agents, County
" Departments of Social Services,and
WHEREAS, County is authorized to purchase certain services for eligible children under State Department
• rules, and
WHEREAS, certain Federal and State changes regarding Medicaid and related rates are occurring and have
not been resolved;and
WHEREAS,the stability of pt'cements of children and youth under the guardianship of the County is vital
and
WHEREAS,County and the Contractor wishes to amend its original agreement by••rrndirg the dur?tiap r f .d

WHEREAS,Contractor is licensed as a Residential Treatment Center.
NOW THEREFORE,it is hereby agreed that in consideration of the mutual Undertakings County and
Contractor agree as follows:
1. The term of the Agreement shall be extended from June 30,2005,to September 31,2005.
2. This Amendment,together with the original Agreement,constitutes the entire understanding among the par ies
with respect to the subject matter hereof, nd may not be changed or modified except as stated in tlgriginrtl
