RE: APPROVE LETTER OF AGREEMENT FOR HEALTH PROMOTION PROGRAMS AND
AUTHORIZE CHAIR TO SIGN -JOHN SNOW INCORPORATED(JSI) RESEARCH AND
TRAINING INSTITUTE
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Letter of Agreement for Health Promotion
Programs between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and John Snow Incorporated (JSI) Research and Training Institute, with terms and
conditions being as stated in said agreement, and
WHEREAS,after review,the Board deems it advisable to approve said agreement, a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,that the Letter of Agreement for Health Promotion Programs between the County
of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Weld County Department of Public Health and Environment, and John Snow
Incorporated (JSI) Research and Training Institute be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreement.
The above and foregoing Resolution was,on motion duly made and seconded, adopted by
the following vote on the 27th day of July, A.D., 2005.

SUBJECT: Agreement with JSI for data analysis
Enclosed for Board review and approval is the agreement between the Weld County
Department of Public Health and Environment and John Snow Incorporated (JSI) for
development of evaluation criteria for Health Promotion Programs and analysis of 2004
Behavior Risk Factor Surveillance (BRFSS) over sample data for Weld County.
BRFSS data collection on adults over the age of 18 in Weld County is funded by the
Steps to a Healthier US/Weld County grant from the Colorado Department of Public Health and
Environment. The data includes information regarding diabetes, asthma, tobacco use, height and
weight, diet and physical activity that will help measure the effectiveness of the project.
The BRFSS data will be collected for the next three years by the Steps grant. The funds
for the additional analysis of the data are included in the 2005 Health Promotion Program
budget. This analysis will provide the health department with baseline information on oral
health, immunizations, male and female health issues, family planning, heart attack and stroke,
and home environment in Weld County.
Your approval is recommended.

Please find enclosed our proposal to Weld County Department of Public Health and
Environment to provide logic models and BRFSS data analysis for Health Promotions Programs.
The anticipated cost of the logic model development and BRFSS data analysis as outlined in the
enclosed proposal is $7,176. This includes consulting expenses as well as any travel expenses to
the Health Department for meetings incurred.
JSI will invoice the Weld County Department of Public Health and Environment, Health
Promotion Program when each task is completed. When the last logic model is completed and
submitted to the Program, JSI will invoice for these services and will invoice when BRFSS data
analysis is completed.
