By December 31,2006,expand the existing social norming media campaign to educate Weld County residents about the abstinence message. c. Impact Objectives i. By December 31,2006,at least 83%of Weld County school districts will be exposing their students to the A-H compliant curriculum,WAIT Training and/or The Art of Loving Well. ii. By December 31,2006, facilitate sexual cessation counseling to 50%of teenage clients who participate in the STD and/or Family Planning Clinic at the WCDPHE. iii. By December 31,2006,at least 25%of the teenagers that receive sexual cessation counseling at the WCDPHE will participate in additional abstinence and relationship education. 4. Briefly describe the target population(s). Weld County,the third largest county in Colorado,is located in the Northeastern part of the state. It covers 3,999 square- miles and in the year 2003 inhabited an estimated population of 207,120. a. Our primary target population is the teenagers in the 12-18 year old age group in Weld County. The population projection for this age group,for 2005,was 23,492. There are 12 school districts with 14 high schools and 16 middle schools,hosting 70.5%white and 27%Hispanic youth,of which many are high risk. b. Our secondary target population groups would be the following: parents,teachers,youth pastors,healthcare professionals and other community youth program leaders. Although these are secondary target groups,the main intention for making them aware of the problem of teenage sexual involvement and the importance of abstinence and relationship education is for the benefit of our primary target population. Briefly describe the program's services.IF the program has changed curricula, format or major elements for implementation in Year Three,please provide details.(Additional pages may be used to provide supplemental information) a. For implementation in year three,our program curricula,format and major elements will remain basically the same. i. We will continue our outreach to the schools throughout Weld County with the WAIT Training and The Art of Loving Well A-H compliant curriculum. ii. We will continue implementing a Rites of Passage community based educational program to promote abstinence to Hispanic adolescent females and their families. iii. We will continue with the parent education presentations and workshops to provide parents with skills to communicate to their children and receive up-to-date sexuality information. iv. We will continue to host events for teenagers and parents during Abstinence Awareness Week and Talk to Your Kid about Sex Month. v. We will continue our sexual cessation counseling within the clinical setting at the WCDPHE. vi. We will continue to reach beyond the schools and reach more youth serving organizations with the abstinence message. vii. We will continue to expand the existing media campaign to saturate Weld County. b. For implementation in year three,our program will expand in the following areas. i. The program will focus more on sustaining and maintaining those teachers who are already trained and providing them with more technical assistance instead of training more individuals. Instead of hosting trainings,our time and energy will go towards encouraging trained teachers to implement more participant contact hours. ii. The program will increase the sexual cessation nurse to .50 FTE to increase the importance of abstinence education and sexual cessation beyond the WCDPHE to other medical facilities within Weld County. iii. The program will encourage and provide more follow-up for teenagers who receive sexual cessation counseling. iv. The program will concentrate on providing more services in the southern part of Weld County. v. The program will engage in more contract work to reach out to the college freshman and the Latino population with the abstinence message and sexually transmitted disease education. 6. Briefly describe how the program will monitor and evaluate its impact and process objectives. The Weld County Department of Public Health and Environment will consult with Cindy Kronauge of Prime Solutions, Ft.Collins,CO,as the Abstinence Education Program's evaluator. Abstinence staff will assist the program evaluator in data collection and management activities.All evaluation information collected will be anonymous. There is currently a pre/post questionnaire used in Weld County to assess the success of the curriculum in schools. Additionally,there will be a form in both English and Spanish to evaluate the Rites of Passage Program. Data is also collected from parents and students after each presentation. We will create and distribute a survey to the sexual cessation counseling participants in order to determine the success of this relatively new piece that we will be implementing. All of the monitoring and evaluation documents will be kept in a binder at WCDPHE. Anonymous client data will be collected through unduplicated client counts and will be tabulated and stored electronically by the abstinence staff and evaluator. The evaluator will prepare a year-end summary report of all the evaluation information. 7. Will there be any changes in program staffing? If so,please provide details. 0 Yes O No The Weld County Abstinence Education Program plans on increasing the FTE for the sexual cessation nurse who is counseling sexually active teenagers toward renewed abstinence. This increase will allow for the program to expand upon the foundation that was established in 2005. With the increase in FTE,the sexual cessation nurse will be able to provide more follow-up to the teenagers. This will enable the teenagers to receive more contact hours and hopefully reach the targeted amount of 15hours of abstinence and relationship education. The more education that is received by the teenagers,the more concrete their decision becomes. The increase in FTE will also enable the sexual cessation nurse to promote sexual cessation counseling to other medical providers in Weld County. Our goal is to take the model that is working so well at WCDPHE and implement it in a variety of youth serving medical facilities through out Weld County. Budget justification: TOTAL Project Cost=$190,072.70 Direct Costs=$176,988.20 i. Personal Services and Fringe Benefits—$148,309.32 Currently,there are three positions,two .75 FT health educators and a.25 FTE sexual cessation nurse employed with the Abstinence Education Program There is an additional.05 supervisor and.05 administrative assistant that spend time on abstinence education projects. The funding for personnel, including fringe benefits and$18,500 for contractual services for the evaluator,UNC Initiative and STD/Abstinence Education Platicas, is provided as matching from the WCDPHE. Our projected goal for 2006 is to increase the sexual cessation nurse from.25 FTE to.50 FTE. $116,372.70 provided by Weld County Board of County Commissioners(matching). $31,936.62 requested from CDPHE for .50 FTE sexual cessation nurse. ii. Rites of Passage Program—$2750 Assisting at least three community partners in implementing a Rites of Passage Program targeted to 60 Hispanic females ages 11-15 and their families. Five new and revised leader's guides @$150 X5 are a total of$750. Meeting expenses($1,000)will help assist community organizations with the cost of the logistical expenses that are involved with implementation of meetings. Class materials and educational incentives will help provide handouts and other abstinence items that the girls can take home as a remembrance of the education that was received($1,000). Funds requested from CDPHE. iii. WAIT Training Technical Support—$300 To host 3 teacher in-service luncheons with 10 participants at each luncheon would be a total of 30 teachers @$10 per teacher for lunch costs$300. Funds requested from CDPHE. iv. Sexual Cessation—$1500 Promotion and incentives($1000)for the support groups include the cost to create reminder cards and provide some educational incentives to encourage clients to engage in more abstinence and relationship education that is offered by the abstinence staff at the schools in Weld County. Meeting expenses will enable the abstinence staff to provide lunch-n-learns and other education opportunities for the clients ($500). Funds requested from CDPHE. v. Abstinence Awareness Week Events—$2000 Motivational,in-state speaker @$1000 for speaker fee/expenses for middle and/or high school assemblies and other youth serving organizations plus$1000 for meeting expenses and educational incentives. Funds requested from CDPHE. vi. Community Empowerment—$7500 To create a variety of different community wide abstinence activities,we plan to collaborate with different community groups and provide a variety of community events that focus around the abstinence message--$1000(i.e. Mother-Daughter Modesty Day,Purity Ball,Prom Prep). To supply 100 youth from different organizations with scholarships to the Pure by Choice Conference is 100 X $10 for a total of$1000. Booth rental fee of up to$500 in order to promote Weld County resources to teenagers who attend the Pure by Choice Conference. Motivational,out-of-state speaker @$4000 for speaker fee/expenses plus$1000 for incentives will be used to reach the students who attend the Latino/Latina Youth Conferences. Funds requested from CDPHE. vii. Weld County Media Campaign—$4105.88 $4105.88 will supply us with the power to implement a media campaign throughout Weld County. We will expand and update our website($500). We plan to display the abstinence message through a variety of different sources such as theatre slides($3000),newspapers($500)and posters in several different youth organizations($105.88). Funds requested from CDPHE. viii. Abstinence Education Incentives/Promotional Items—$1000 To supply the teenagers of Weld County with abstinence educational incentives and promotional items that will serve to positively influence theft peers toward the abstinence message. We plan to have items to educate the students after our presentations. The program will also give teachers incentives to continue to encourage their students throughout the semester. We will also use abstinence incentives to boost referrals from nurses and engage participants in technical assistance sessions. Examples: brochures,posters,t-shirts, stickers,temporary tattoos,water bottles,pens/pencils,highlighters,mints, suckers and bookmarks. Funds requested from CDPHE.
