RE: APPROVE REVISION TO CORE SERVICES AND CHAFEE FOSTER CARE
INDEPENDENCE PROGRAM PLAN
WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Revision to the Core Services and Chafee
Foster Care Independence Program Plan,approved by Resolution#2004-1273,between the County
of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Department of Social Services, and Colorado Department of Human Services,
Division of Child Welfare Services,with further terms and conditions being as stated in said revision,
and
WHEREAS,after review,the Board deems it advisable to approve said revision, a copy of
which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Revision to the Core Services and
Chafee Foster Care Independence Program Plan between the County of Weld,State of Colorado,
by and through the Board of County Commissioners of Weld County,on behalf of the Department
of Social Services, and Colorado Department of Human Services, Division of Child Welfare
Services, be, and hereby is, approved.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 28th day of March, A.D., 2005.

RE: Revision to Core Services and Chafee Foster Care Independence
Program Plan for FY2003-2005
Enclosed for Board approval is a revision to the Core Services and Chafee Foster Care
Independence Program Plan for FY2003-2005. The revision to the plan was discussed and
reviewed at the Board's Work Session of March 9, 2005.
The major revision is to shift existing personnel expenditures from the Child Welfare Block Grant
to the Core Services Plan. This effort ensures that monies are first exhausted under the Core
Services Plan before Weld County's under expenditures are used as close out resources for other
counties.

7.303.1 C Life Skills: Services provided primarily in the home that teach household management, effectively accessing
community resources, parenting techniques, and family conflict management.
Indicate information for each line service worker and his/her immediate supervisor for whom Core Services funding is
proposed in whole or in part. Include only amounts that are to be charged to Core Services. Staff positions to be included are
County Core Services employee positions and employee contract positions.
