RE: APPROVE APPLICATION FOR COMMUNITY HEALTH INVESTMENT PROGRAM -
PLATTE VALLEY MEDICAL CENTER
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Application for Community Health
Investment Program from the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Human Services, Area
Agency on Aging,to the Platte Valley Medical Center,with terms and conditions being as stated in
said application, and
WHEREAS,after review,the Board deems it advisable to approve said application, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,that theApplication for Community Health Investment Program from the County
of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Department of Human Services,Area Agency on Aging,to the Platte Valley Medical
Center be, and hereby is, approved.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by

Dental, Hearing and Vision Program Funds from the Platte Valley
Medical Center's Community Investment Program.
Enclosed for board approval is the Area Agency on Aging's funding request of$10,000 from
the Platte Valley Medical Center's Community Investment Program. The funds will be used to
enhance the Area Agency on Aging's Dental, Hearing and Vision services to seniors 60 years
of age and older who reside in south Weld County.

This application, with at least one additional proposal page, and a copy of the following must be submitted
to the CHIP Committee:
1) Names and addresses of applicant officers and members of its Board of Directors or governing body.
2) Articles of Incorporation and By-laws or other documents which define the applicant's purposes, membership,
management and operation.
3) Statute, regulation or the applicant's Internal Revenue Service letter which shows the applicant is exempt from
Federal income taxes, is not a private foundation and that donors may deduct contributions to the applicant as
provided in Section 170 of the Internal Revenue Code.
4)A financial audit or other financial information for the most recent three fiscal years.
5)A detailed budget for the proposal.
All applications must be submitted to the CHIP Committee no later than the month ending previous
to scheduled CHIP Committee meetings (The Committee meets the second Wednesday every other
month as follows: January, March, May,July, September and November).
COMMUNITY HEALTH INVESTMENT PROGRAM GRANT REQUEST
FOR PROGRAM YEAR BEGINNING AUGUST 1, 2005 AND ENDING
JULY 30,2006.
PROPOSAL:
The Weld County Area Agency on Aging has received three grants from the CHIP
Committee to expand our dental, hearing and vision program into South Weld
County. When we submitted the first grant request, we believed that here was a
need for these very important services in South Weld County. We had received
funding from State, but did not have sufficient funds to adequately meet the needs
of eligible seniors in South County. With the funding we have received through the
CHIP Committee, we have been able to supplement matching state funds to allow
our dental, vision and hearing program to expand into South County. As before,
the funding we are scheduled to receive from the State and with funding from the
CHIP Grant, we wish to continue to offer the dental, hearing and vision services
throughout Weld County. Statistically, data cited in Older Americans 2004: Key
Indictors of Well-Being supports the belief that this type of program is greatly
needed to meet the needs of the seniors by providing financial assistance to enable
seniors to obtain the services needed to maintain their health and a quality of life.
It has been our experience that seniors requesting assistance may need a
combination of services such as dental and vision.
DOCUMENTED NEED:
We have found that the need for dental, hearing and vision services continues to
exist in South Weld County. We continue to use the assistance of the Fort Lupton
Senior Center, Salud Family Health Clinic and have other agencies, including area
churches, in South Weld County, to provide information to seniors regarding the
dental, hearing and vision program. Two additional area providers have been
added to ensure easier access to program services; the Vision Center at Wal-Mart
in Longmont and Heather Romano, OD in Longmont.
Seniors in South Weld County have been instrumental in notifying other seniors
about the services available. We believe there are still seniors in need of services in
South Weld County, and will do all we can to promote the program to these seniors.
We continue to receive requests for information on the dental, hearing and vision
services from adult children whose parents reside in South Weld County. Seniors
who have been successfully assisted with one of the three services in the past, find
they need additional services contact us. This is a clear indication of how important
these services are to seniors. The CHIP Grant increases the possibility of serving
these seniors.

EXPECTED RESULTS OF THE PROGRAM:
For Program Year 2002-2003, the Weld County Area Agency on Aging Dental,
Vision and Hearing Program, set a goal to assist twenty three (23) seniors within the
dental, vision and hearing program with the $ 20,000 grant funds we received from
the CHIP Committee for the Program Year. The participant numbers were higher
than expected, twenty eight (28), because we were able to work with service
providers that were willing to offer discount to seniors, thus allowing more
participants to be eligible for assistance.
Program Year 2003-2004 we set a goal to provide service to thirty (30) seniors. As
of the end of March, you will recall, eighteen (18) seniors were provided service with
thirteen (13) participants that had been approved for assistance and were in the
process of receiving services. A total of thirty-one (31) recipients were achieved.
Program Year 2004-2005 the goal set was to provide service to thirty (30) seniors.
By the end of February 2005, twenty-four (24) South Weld County residents have
been found eligible and funds committed for assistance. We anticipate that within
the next four months, we will be able to assist four (4) more individuals. We have
found that in five (5) situations that eligible seniors needed a combination of services
to adequately address their needs such as vision and dental. It is important to note
that currently a waiting list exists for seniors who reside in North Weld County. We
anticipate a waiting list for South County seniors as well.
SPECIFIC OBJECTIVES, TIMETABLES AND PROCESS UTILIZED TO
RESEARCH PROJECT GOALS:
For Program Year 2005-2006, our goal is to enable seniors to obtain the services
needed to maintain their health and a quality of life. To reiterate, we have found
that in five (5) situations that eligible seniors needed a combination of services to
adequately address their needs. We hope to assist thirty (30) seniors in South Weld
County 2005-2006. Continued coordination with aforementioned entities we believe
will prove to be effective in marketing the dental, vision and hearing aid program.
We will continue to encourage local doctors (dental, vision and hearing) to make
referrals for seniors who may need assistance.
HOW WILL RESULTS BE MEASURED?
The participants and services provided through these programs are tracked using
SAMS 2000, our agency software. This software allows us to continue to submit
progress reports as required by CHIP Committee. We will continue to seek
feedback from clients as to their level of satisfaction for services received.
Due to the funding received from the CHIP Committee, we have been able to
expand our dental, hearing and vision program in South Weld County. We have
contacted providers in the area who have a genuine interest in assisting seniors in

need of the services that we are offering. With the funds provided by the CHIP
Committee as supplement funds, and dollars received from the State and the Older
Americans Program, these past three Program Years, the dental, vision and
hearing program in South Weld County have been most successful. We are
enabling seniors to obtain the services needed to maintain their health and a quality
of life.
We are asking the CHIP Committee members, to once again partner with us in
continuing to meet the needs of South Weld County seniors, Program Year 2005-
2006. Any funds received from the CHIP Committee will be used as supplemental
funds. The State General Funds will continue as our primary source of funding for
the Dental, Vision and Hearing Program.

The five member Board of County Commissioners is the statutory head of Weld County's governme
of the commissioners are elected at large and the other three are elected from the district in which
reside. As is the case with all elected county officials, commissioners must be at least 21 years of a
have resided in the county for at least one year prior to their election. Their elected position must t
primary employment during the term they serve.
As a board, the commissioners are required to establish a budget, levy taxes, appropriate funds for

business, establish fees and licenses, purchase or otherwise acquire, own, sell, lease, trade, and of
deal in real and personal property in the name of the county, receive gifts and grants for the count
approve and execute contracts and lawsuits in the name of the county.
The board also is required to sit as a board of appeals to hear complaints on actions taken by couni
boards, commissions and departments, as a board of equalization to hear appeals on tax assessme
a county board of welfare, and to make appointments to fill vacancies in all county offices except tt
board of commissioners and the county council.
Each commissioner has primary responsibility for one of the county's five departments and serves
assistant in another. The five departments are communication services, health services, planning si
engineering services, and finance, administration, central purchasing, and personnel. Their respons
rotate each year.
The chairman of the Board of County Commissioners is chosen by the commissioners each year anc
always serves as chairman of the finance and administration department during his tenure as chair
the board.
Commissioners are only allowed to serve two complete elected terms under charter provisions.
The board meets at 9:00 a.m. each Monday and Wednesday in the first floor meeting room at the
Centennial Center Complex, 915 10th Street, Greeley.
