HomeMy WebLinkAbout20052001.tiff RESOLUTION RE: APPROVE TASK ORDER OPTION TO RENEW LETTER #1 FOR WEST NILE VIRUS AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS,the Board has been presented with Task Order Option to Renew Letter#1 for West Nile Virus between the County of Weld,State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment,commencing July 1, 2005, and ending June 30, 2006,with further terms and conditions being as stated in said task order, and WHEREAS, after review, the Board deems it advisable to approve said task order, a copy of which is attached hereto and incorporated herein by reference. NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that Task Order Option to Renew Letter#1 for West Nile Virus between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said task order. This option to renew letter will provide funding of$54,867 for the time period July 1, 2005 through June 30, 2006. The funding will be used for personnel, travel, laboratory supplies, field surveillance and equipment, and supplies for public health education on West Nile Virus prevention. The funds are passed through the CDPHE from the Centers for Disease Control. I recommend your approval of this task order option to renew letter. Enc. 2005-2001 TASK ORDER OPTION TO RENEW LETTER June 21,2005 Task Order Option to Renew Letter Number 1, Contract Routing Number 06 FHA 00063 State Fiscal Year 2006-2006 Disease Surveillance Program This Task Order Option to Renew Letter is issued pursuant to paragraph E4 of the Master Contract identified by contract routing number 05 FAA 00054 and paragraph C.5.of the Task Order identified by contract routing number 05 FHA 00098 and contract encumbrance number PO FHA EPI0500098. This Task Order Option to Renew Letter is between the COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT and the BOARD OF COUNTY COMMISSIONERS OF WELD COUNTY. The Task Order has been amended by Task Order Change Order Letter 1,contract routing number 05 FHA 00166. The Task Order, as amended,if applicable, is referred to as the"Original Task Order". This Task Order Option to Renew Letter is for the renewal term of July 1,2005,through June 30,2006. The maximum amount payable by the State for the work to be performed by the Contractor during this renewal term is FIFTY FOUR THOUSAND EIGHT HUNDRED SIXTY SEVEN DOLLARS,f$54,867.00)for an amended total financial obligation of the State of ONE HUNDRED FIFTEEN THOUSAND FIVE HUNDRED SEVENTY DOLLARS. Method of accountability: submission of copies of receipts and documentation for personnel, purchase of equipment and educational activities. Provide animal surveillance data, conduct investigation of human cases and report to CDPHE.
