Project: Weld County Bridge WEL027.0-058.0A over Sheep Draw. The project is located on Two Rivers Parkway or
83` Ave.just south of West 10th St.,Greeley,CO and is funded with a Federal Bridge Replacement Grant. Sear-Brown
was selected as the consultant for the project to provide engineering design and construction management services.

The following change is hereby made to the Contract Documents:
o Additional services to the original Scope of Work were required. Additional services included:
• Design sidewalk under bridge to facilitate future City of Greeley Sheep Draw Trail $1,200.00
(City of Greeley has agreed to reimburse Weld County for these services.)
• Redesign of concrete deck and as built drawings of deck and girders $12,000.00
(Costs to perform services will be deducted from the contractor's contract due to incorrect survey of
abutment and pier elevations which required adjustments in deck elevations.)
All costs for Change Order No. 4 will be reimbursed to Weld County either by City of Greeley or Hamon Contractors and will
not increase Weld County's overall cost for the total project.
CHANGE TO CONTRACT PRICE:
Original Contract Price: $ 169,718.00
Current Contract Price adjusted by previous Change Order: $ 186,206.67
The Contract Price due to this Change Order will be increased by: $ 13,200.00
The new Contract Price, including this Change Order,will be$ 199,406.67
CHANGE TO CONTRACT TIME:
The Contract time will be increased by 0 calendar days.

SUBJECT: Item for Agenda
Please submit the following for the Board's next agenda:
Change Order No. 4 for Weld County Bridge 27/58A over Sheep Draw.
Weld County has contracted with Stantec for design and construction inspection services for
the bridge replacement project for the amount of $169,718.00. Previous approved change
orders increased the amount to $186,206.67.
Attached is Change Order No. 4. This change order will increase the contract amount by
$13,200.00 for additional services that were outside the original scope of work. The additional
services include the following:
• Design of sidewalk under bridge to facilitate future City of Greeley's Sheep Draw
Trail. The cost to provide these services are $1,200.00 (City of Greeley has
agreed to reimburse Weld County for the cost of the design)
• Additional design services for the concrete deck and draft as-construction drawings
for the changes to the deck and girders. The additional services were required
due to an incorrect survey of the south abutment and the pier elevations performed
by the contractor's surveyor. The cost of $12,000.00 will be deducted from the
contractor's contract to reimburse the project for the additional services;therefore,
no cost to the overall project.
The new contract amount will be $199,406.67. The total cost of Change Order No.4 will
be reimbursed to Weld County by the City of Greeley and Hamon Contractors. The
project is scheduled to be completed mid July.
