RE: APPROVE AMENDMENT OF CHILD PROTECTION AGREEMENT FOR SERVICES
AND AUTHORIZE CHAIR TO SIGN - ISLAND GROVE REGIONAL TREATMENT
CENTER, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment of Child Protection
Agreement for Services between the County of Weld,State of Colorado,by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and
Island Grove Regional Treatment Center,Inc.,commencing December 1,2004,and ending May 31,
2005, with further terms and conditions being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment,a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Amendment of Child Protection
Agreement for Services between the County of Weld,State of Colorado,by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and
Island Grove Regional Treatment Center, Inc. be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be,and hereby is,authorized to
sign said amendment.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 6th day of April, A.D., 2005, nunc pro tunc December 1, 2004.

Agreement Between the Weld County Department of Social
Services and Island Grove Regional Treatment Center
Enclosed for Board approval is an Amendment of Child Protection Agreement for Services
between the Weld County Department of Social Services (Department) and Island Grove
Regional Treatment Center(Island Grove). This Agreement was reviewed at the Board's Work
Session of April 4, 2005.
The major purposes of the Amendment are to increase funding for Island Grove from$120,000 to
$195,000 to pay for treatment services for child welfare clients and to include fee schedule for
Swab testing and confirmation services.
All other provisions of the original Agreement remain the same including the term beginning
December 1, 2004 through May 31, 2005, the fee schedule, and the funding source of Child
Welfare Administration or Core Service Funds.

CHILD PROTECTION AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF SOCIAL SERVICES
AND ISLAND GROVE REGIONAL TREATMENT CENTER,INC.
This AMENDMENT to Child Protection Agreement for Services,made and entered into the
th day of April 2005,by and between the Board of County Commissioners, sifting as the Board of
Social Services, on behalf of the Weld County Department of Social Services,hereinafter referred to as
"Social Services"; and Island Grove Regional Treatment Center,Inc.,hereinafter referred to as"Island
Grove."
WITNESSETH
WHEREAS, Social Services and Island Grove entered into a Child Protection Agreement for
Services (Contract Number 04-CPS-33) on January 10, 2005, to provides services of a substance abuse
treatment provider to assist social Services in delivering substance abuse treatment services to child welfare
clients upon the depletion of Core Funding and Additional Funding Services(AFS)funding;and
WHEREAS, the monies provided under the agreement is not sufficient for the term of the
Agreement(Contract Number 04-CPS-33)to serve the treatment needs of child welfare clients; and
WHEREAS, Social Services desires to adjust its payment schedule to respond to the increased
needs of child welfare clients.
NOW, THEREFORE, in consideration of the premises, the parties hereto covenant and agree as
follows:
1. Exhibit A,Scope of Services, Item 1, Service Fee Schedule, shall be amended to include
Swab testing at$15.00 each and Swab confirmation on positive tests at$45.00 each.
2. Exhibit B,Payment Schedule,Item 1,Funding and Method of Payment, shall be
amended as follows: Social Services agrees to reimburse to Island Grove in
consideration of the work and services performed,a total not to exceed One Hundred
Ninety-five Thousand Dollars($195,000.00)including a 5%administrative fee not to
exceed$11,700 and under Child Welfare Regular Administration funding or Core
Services Funding,as deemed appropriate by Social Services.
3. This Amendment, together with the original Child Protection Agreement for Services
constitutes the entire understanding among the parties with respect to the subject matter
hereof, and may not be changed or modified except as stated in the Child Protection
Agreement.
