RE: APPROVE FOSTER CARE AND ADOPTION GRANT REQUEST AND AUTHORIZE
CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Foster Care and Adoption Grant Request
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Social Services, and the Colorado
Department of Human Services, with further terms and conditions being as stated in said
application, and
WHEREAS, after review, the Board deems it advisable to approve said application, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Foster Care and Adoption Grant
Request between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County,on behalf of the Department of Social Services, and the Colorado
Department of Human Services, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said application.
The above and foregoing Resolution was,on motion duly made and seconded, adopted by

RE: Approval of Foster Care and Adoption Grant Request
Enclosed for Board approval is a Foster Care and Adoption Grant Request. This grant request
was reviewed at the Board's Work Session of June 27, 2005.
The Department was informed of a $4,000 grant that was available to Weld County on June 14,
2005. The Colorado Department of Human Services requested a plan to use these monies with
the provision that these monies needed to be encumbered and received by June 30, 2005. These
monies do not require a local match. The initial plan was approved by the Colorado Department
of Human Services on June 20, 2005. The Department requests the Board's approval to its grant
request and authority to proceed with the use of these grant funds.

The Board of County Commissioners, Weld County, authorizes the Weld County
Department of Social Services to apply and to receive funding by June 30, 2005, in the
amount of$4,000 for purposes of services and resources for foster care and adoptive
families in Weld County. These monies are Federal and State resources and do not
require 20% county matching dollars. The description of items to be purchased under the
$4,000 grant is outlined in a June 14, 2005, letter to Colorado Department of Human
Services, which is attached for reference. The contents of such letter have been approved
by the Colorado Department of Social Services.
