RE: APPROVE AMENDMENT #1 TO THE WORKFORCE CONTRACT AND AUTHORIZE
CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with Amendment#1 to the Workforce Contract
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, Employment
Services of Weld County,and the Department of Local Affairs,on behalf of the Office of Workforce
Development,and the Colorado Workforce Development Council,with terms and conditions being
as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment,a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Amendment#1 to the Workforce Contract between the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County,on behalf
of the Department of Human Services, Employment Services of Weld County, and the Colorado
Department of Local Affairs, on behalf of the Office of Workforce Development and the Colorado
Workforce Development Council be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 20th day of June, A.D., 2005.

SUBJECT: Amendment to Contract between the
Department of Local Affairs and
Employment Services of Weld County
Enclosed for Board approval is Amendment #1 to the Contract between Colorado
Department of Local Affairs and Weld County/Employment Services of Weld County for
a Disability Program Navigator. This amendment increased the contract funding from
$39,550 to $89,050, for the period of September 1, 2004 through June 30, 2006.

CRS 24-30-202 requires that the State Controller approve all state contracts. This contract is not valid until the State
Controller, or such assistant as he may delegate, has signed it. The contractor is not authorized to begin performance

State and Contractor Executed Modifications
A. Modifications to Contract Boilerplate
Compensation and Method of Payment:
"Compensation and Method of Payment" in the Original Contract is modified by deleting "
$39,550 " and inserting in lieu there of" $89,050 .
B. Modifications to Exhibit A, Scope of Services.
Project Description, Objectives and Requirements: "Project Description, Objectives, and
Requirements, Subsection 1." is modified as follows:
"Contractor will support, manage, recruit and employ a FTE Disability Program Navigator.
Contractor will ensure that the Navigator performs the basic job functions included in the navigator
job description provided by USDOI,/ SSA.
Contractor will manage and oversee the effective delivery of these services either through their
direct employee or a contracted employee of an acceptable entity and will provide a work station
consistent with the center's policy.
Contractor will provide the funding and time available for the Navigator to attend a 4-day training in
Washington DC. Also ensure the Navigators attendance in 8 of the 10 monthly conference calls.
Contractor shall keep appropriate records as required by Department of Local Affairs (DOLA).
Contractor will co sponsor and promote attendance of seminars and forums held in its workforce
region and will actively recruit business, PWD, organizations that serve PWD and workforce board
members to ensure successful attendance.
Contractor agrees to cooperate with OWD, its technical assistance experts and staff in establishing a
seamless, comprehensive employment service plan and implementation in the workforce center or
centers identified in its agreement with OWD.
In addition to the above, the Contractor will:
Prudently administer funds assigned to its care through this contract.
Work with OWD staff to provide training to parents and educators about youth (who are considered
PWD) transitions from school to work.
Submit best practices to OWD and the best practices E learning website.
Provide sufficient space and support services for Benefits Planners in its workforce centers.
Provide referrals to Benefits Planners for PWD that have technical questions regarding disability
laws, rules, regulations of the SSI/SSA.
Review and assess center capabilities regularly or as requested by OWD.
Participate in USDOL/ODEP surveys and data collection; including the submission of a quarterly
report as well as ensuring all WFC staff are completing the additional fields established in Job Link
(type of disability and type of referrals made for individuals self identifying as having a disability).
Collaborate with Division of Vocational Rehabilitation, Department of Labor& Employment and
other consortium partners to coordinate provision of service from different funding streams for
PWD".
Project Description, Objectives and Requirements is revised to read:
"Contractor will support, manage, recruit and employ 1 FTE Disability Program Navigator.
Contractor will ensure that the Navigator performs the basic job functions included in the navigator
job description provided by USDOL/ SSA. Contractor acknowledges all Navigators must be located
in One-Stop Career Centers that are universally accessible to individuals with disabilities, including
physical, programmatic, and communications accessibility.
Contractor agrees that a Navigator is:
o Not a case manager
o Not a front-line staff person in a One-Stop center
o Not a Benefits Planning and Outreach(BPAO) Specialist
o Not a Vocational Rehabilitation Specialist
Contractor demonstrates that they understand that a Navigator is a:
o resource to the One-Stop Career Center staff
o facilitator
o problem solver
o systems change agent
o relationship builder
Contractor agrees to the following service and outcome performance goals for the DPN initiative
required under the Government Performance and Results Act as follows:
✓ 6% of participants served by the workforce investment area(s) receiving grant funds
will be persons with disabilities.
✓ The entered employment rate for participants with disabilities that exit the WIA adult,
dislocated worker, and youth programs will be 70%

The State shall reimburse the contractor's reasonable, allowable costs, as defined herein,
not exceeding Eighty nine thousand fifty Dollars ($89,050).
All of the terms and conditions of the Original Contract remain unchanged except for those
terms and conditions modified by this Amendment# 1 and all previous amendments. Both
parties also expressly understand that this Amendment # 1 is incorporated into the Original
Contract.
