RE: APPROVE PURCHASE OF SERVICES AGREEMENT AND AUTHORIZE CHAIR TO
SIGN -JACQUELINE JOHNSON
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Purchase of Services Agreement between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Social Services, and Jacqueline Johnson, commencing
July 1, 2005, and ending June 30, 2006, with further terms and conditions being as stated in said
agreement, and
WHEREAS,after review,the Board deems it advisable to approve said agreement, a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Purchase of Services Agreement
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Social Services, and Jacqueline
Johnson be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreement.
The above and foregoing Resolution was,on motion duly made and seconded, adopted by
the following vote on the 29th day of August, A.D., 2005, nunc pro tunc July 1, 2005.

RE: Purchase of Services Agreement—Jacqueline Johnson
Enclosed for Board approval is a Purchase of Services Agreement between the Weld County
Department of Social Services (Department) and Jacqueline Johnson. This Agreement was
reviewed at the Board's Work Session held on June 20, 2005.
The major provisions of the Agreement are as follows:
1. The term of the contract is July 1, 2005 through June 30, 2006.
2. The source of funding is Colorado Works or TANF funding.
3. Jacqueline Johnson has been interviewed, approved, and selected as its Facilitator by the
Interagency Oversight Group, which was formed under the Colorado Statute 14-1.9-101
(HB04-1451), Collaborative Management of Multi-agency Services Provided to
Children and Families. Ms. Johnson will assist the Interagency Oversight Group in its
efforts to meet its planning goals as described in its Memorandum of Understanding.
4. The Department agrees to reimburse Ms. Johnson for professional services to not exceed
$40,000 a year.

PURCHASE OF SERVICES AGREEMENT
THIS AGREEMENT is made and entered into this day of ,2005,by and between the Board
of County Commissioners of the County of Weld, State of Colorado, sitting on behalf of the Weld County
Department of Social Services, hereinafter referred to as"Social Services", and Jacqueline Johnson,hereinafter
referred to as"Facilitator".
WITNESSETH:
WHEREAS, Social Services is a party to a Memorandum of Understanding, originally entered into on May
31,2005,with multiple agencies, divisions,and units of departments at the State and county levels; and
WHEREAS,the parties to the Memorandum of Understanding desire to entered into an agreement for the
collaboration of services families and children who would benefit from integrated multi-agency services as provided
by Colorado Statute 14-1.9-101, Collaborative Management of Multi-agency Services Provided to Children and
Families; and
WHEREAS,the Facilitator has the expertise and knowledge to assist the parities through its year long
planning process as provided in its Memorandum of Understanding.
NOW, THEREFORE, in consideration of the mutual promises and covenants contained herein,the parties
hereto agree as follows:
1. TERM: The term of this Agreement shall be from July 1, 2005, through and until June 30, 2006.
2. SERVICES PROVIDED BY FACILITATOR: The Facilitator agrees to assist, through
facilitation,the Interagency Oversight Group's efforts to meet its planning goals as described in its
Memorandum of Understanding(MOU)under HB04-1451,as provided in Exhibit A,and
pursuant to Colorado Statute 14-1.9-101,and applicable State rules.
A. Under the direction of the Officers of the Interagency Oversight Group(IOG),the
Facilitator will develop a timeline for implementation and achieve, through consensus
wherever possible among the IOG members,certain planning objectives that will include,
at a minimum, the following elements:
I) The establishment of principles to guide the planning process; and
2) The identification of target populations to be included under the MOU
beginning July 1, 2006 and the identification of the target population needs for
planning purposes;and
3) The identification and definition of outcomes to meet such needs identified of
the target population; and
4) The establishment of definitions of certain terms including collaboration;
balanced continuum of care including assessment and levels of care,and
5) The prioritization of service needs of the target population not being met by
existing resources;and
6) The prioritization of collaboration practices or needs for the target population to
be served; and
7) The prioritization of case management protocols needs required of the target
population and not being met by current practices; and
8) The identification of community standards for evidence-based practices or
standards that may be applied to existing and future services; and
9) The identification of community standards regarding a balanced continuum of

services; and
10) The identification of community standards to document and validate such
evidence-based practices.
B. Concurrent to the services provided by the Facilitator in Item 1,A,the Facilitator will
organize and present to the IOG certain information which would be available through
members of the IOG, CSU, and State Department of Human Services resources for
purposes of the IOG's planning. At a minimum, such information will include the
identification of:
1) Existing collaborative practices,efforts, and committees established for the
target population, identify gaps in collaboration for such target population, and
identify barriers to such collaboration; and
2) Existing services being provided by members of the IOG to the target
population,gaps in services identified by such members, and barriers in
resources,rules, and other areas identified by IOG members; and
3) Existing case management protocols identified among the members o the IOG to
the target population,gaps in case management protocols identified by such
members,and barriers to effective case management in resources, rules, and
other areas identified by IOG members; and
4) Existing roles and responsibilities of all paraprofessionals and professionals
involved with the target population through Dependency&Neglect and
delinquency cases,gaps in responsibilities and/or barriers to perform such roles
and responsibilities; and
5) Existing information through flow charts or other means to present the
progression of the target population through the court process and the laws
associated with such progression, and the identification of any gaps or barriers
met by the target population through the process;and
6) Existing information regarding evidence based practices,promising new
practices, or other ways to quantify or qualify best practices; and
7) Existing data collection sources used by members of the IOG to document and
validate evidence-based practice and the identification of any gaps or barriers to
track the outcomes of the target population.
C. Document the outcomes of the planning process in a report to the IOG regarding Item 2,
A and B in a final report to be prepared by May 31, 2006. Present such report to the
Board of County Commissioners and,upon request,to each IOG member's
respective Board.
3. COMPENSATION: Social Services agrees to pay monthly the Facilitator for professional services
pursuant to this Agreement at the rate of$175 per hour and less than$40,000 of the term of the
Agreement.The parties agree that the service rate as contemplated herein shall constitute the
entire amount of compensation due to the Facilitator for any work performed hereunder.
4. INDEPENDENT CONTRACTOR: Facilitator agrees that it is an independent contractor and will
not become an employee of Social Services,nor is the Facilitator entitled to any employee benefits
under Social Services as a result of the execution of this Agreement.
5. INDEMNIFICATION: Facilitator shall indemnify Weld County, its officers and employees,
against liability for injury or damage caused by any negligent act or omission by Facilitator in the
performance of this Agreement and shall hold Weld County harmless from any loss occasioned as
a result of the performance of this Agreement.

6. NON-ASSIGNMENT: Facilitator may not assign or transfer this Agreement,any interest therein
or claim thereunder,without the prior written approval of Social Services.
7. ACCESS TO RECORDS: Social Services shall have access to Facilitator's financial records as
these records relate to payment and expenditures under this Agreement for the purposes of audit.
Such records shall be complete and available for audit 90 days after final payment hereunder and
shall be retained and available for audit purposes for at least five years after final payment
hereunder.
8. TERMINATION: Either party may terminate this Agreement at any time by providing the other
party with a 10-day written notice thereof Furthermore,this Agreement may be terminated at any
time without notice upon a material breach of the terms of the Agreement.
9. TIME OF THE ESSENCE: Time is of the essence in each and all of the provisions of this
Agreement.
10. ENTIRE AGREEMENT/MODIFICATIONS: This Agreement constitutes the entire understanding
between the parties with respect to the promises and covenants made therein. No modification of
the terms of this Agreement shall be valid unless made in writing and agreed to by both parties.
11. NON-APPROPRIATION:No portion of this Agreement shall be deemed to create an obligation
on the part of Social Services to expend funds not otherwise appropriated in each succeeding year.
12. WAIVER OF IMMUNITIES/THIRD PARTY LIABILITY: No portion of this Agreement shall be
deemed to constitute a waiver of any immunities of Social Services or its officers or employees
may possess, nor shall any portion of this Agreement be deemed to have created a duty of care
with respect to any persons other than Social Services and not a party to this Agreement.
