Weld County Department of Planning Services on my/our behalf for land owned
by me/us described as:
Lot 8 Lupton Meadows Land Co. Div 3 & Reolat Lots 1-8, being Dart of 19,
Township 2 North, Range 66 West of 6th P.M., Weld County, Colorado
I/We authorize the above to obtain information that may be required for said
application from any appropriate agency or institution and I/We hereby authorize
the release of said information for this purpose.
I/We agree to indemnify and hold harmless the preparer of this application for
claims made in this regard. This letter of authorization grants limited power-of-
attorney to the preparer to sign documents on my/our behalf for the purposes of
this application only.

Bearson — Vacation of Subdivision Lot
The purpose of this application is to vacate Lot 8 of the Lupton
Meadows Subdivision (re-plat) from the subdivision. Situate upon the
property is a home and improvements located at 12407 WCR 18, Ft.
Lupton, CO. Just north of the improvements is the South Platte
Supply Canal, which bi-sects the property which is not bridged and
completely eliminates access to the north portion of the parcel.
A request was made to WCDPS to divide the property by the
existence of the South Platte Supply Canal; however there was not
adequate recorded evidence to compel WCDPS to allow that split.
The only other option is to complete a Recorded Exemption to
separate the parcels. In order to complete the RE, it is necessary to
vacate this lot from the subdivision.
The parcel is the eastern-most lot of the subdivision (see map) and
would not create an alcove or orphan lots. No construction is
planned or anticipated at this time, thus the application does not
address any items applicable to such development. No change or
disturbance to the surface is planned. No change in landscaping is
planned.
This application for vacation is submitted concurrently with an
application for Recorded Exemption at the direction of the planners at
WCDPS.

GAP PROTECTION
When this Company conducts the closing and is responsible for recording or filing the
legal documents resulting from the transaction, the Company shall be responsible for
all matters which appear on the record prior to such time of recording or filing.
MECHANIC'S LIEN PROTECTION
If you are a buyer of a single family residence you may request mechanic's lien
coverage to be issued on your policy of insurance.
If the property being purchased has not been the subject of construction,
improvements or repair in the last six months prior to the date of this commitment
the requirements will be payment of the appropriate premium and the completion of an
Affidavit and Indemnity by the seller.

months prior to the date of this commitment the requirements may involve disclosure
of certain financial information, payment of premiums, and indemnity, among others.

CONSUMER DECLARATION STATEMENT
