RE: APPROVE AMENDMENT TO THREE NOTIFICATION OF FINANCIAL ASSISTANCE
AWARDS FOR FOSTER PARENT CONSULTATION PROGRAM WITH VARIOUS
PROVIDERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an amendment to three Notification of
Financial Assistance Awards for the Foster Parent Consultation Program between the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,on
behalf of the Department of Social Services, and various providers, listed below, commencing
June 1, 2004,and ending May 31,2005,with further terms and conditions being as stated in said
amendment, and
1. Ackerman and Associates, PC
2. Lutheran Family Services
3. Transitions Psychology Group, LLC
WHEREAS,after review,the Board deems it advisable to approve said amendment,a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,ex-officio Board of Social Services,that the amendment to three Notification of
Financial Assistance Awards for Foster Parent Consultation Program between the County of Weld,
State of Colorado,by and through the Board of County Commissioners of Weld County,on behalf
of the Department of Social Services, and Various Providers listed above, be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be,and hereby is,authorized to
sign said amendment.

RE: Amendment of Notification of Financial Assistance Awards for the
Foster Parent Consultation Program with Various Vendors
Enclosed for Board approval is an Amendment of Notification of Financial Assistance Awards
(NOFAA) for the Foster Parent Consultation Program with various vendors. The amendment of
the NOFAA was reviewed at the Board's Work Session of April 4, 2005.
The major purpose of the amendment is to adjust the group rate from$350 per group of six foster
parents to a rate of$58.33 per participant per group.
The other aspects of the NOFAA remain the same including the award period of June 1,2004
though May 31, 2005; the source of funding through Core Funds; and restrictions regarding
individual consultation for foster parents.
