RE: APPROVE AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
AND AUTHORIZE CHAIR TO SIGN - COMPLIANCE ADMINISTRATIVE SERVICES
(CAS)
WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with an Amendment to the Child Protection
Agreement for Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and
Compliance Administrative Services (CAS), commencing September 1, 2005, and ending
May 31, 2006, with further terms and conditions being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE,BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,ex-officio Board of Social Services,that the Amendment to the Child Protection
Agreement for Services between the County of Weld, State of Colorado,by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and
Compliance Administrative Services (CAS) be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 21st day of November, A.D., 2005, nunc pro tunc September 1, 2005.

RE: Amendment to Child Protection Agreement for Services Between the Weld
County Department of Social Services and Compliance Administrative Services
(CAS)
Enclosed for Board approval is a Child Protection Agreement for Services between the Weld
County Department of Social Services (Department) and Compliance Administrative Services
(CAS). This Amendment was reviewed at the Board's Work Session of November 2, 2005.
The major provisions of the Agreement are as follows:
1. CAS has recently acquired WCPI/Mayfield,as of September 1, 2005. The Amendment
provides for the transfer of our current Agreement with WCPI/Mayfield to CAS.
2. It amends the cost of two fees. The impacted fees are: Instant 5 panel Drug Screen to
$14.00 and the Standard Laboratory Test to $13.00.
3. The total reimbursement level of the Agreements has been increased from$30,000 to
$45,000.
All other terms of the Agreement remain the same including the original term of the Agreement
of June 1, 2005 through May 31, 2006. The source of funding is from the Child Welfare Block
Grant.

AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF SOCIAL SERVICES AND
COMPLIANCE ADMINISTRATIVE SERVCIES(CAS)
This Amendment, made and entered into the / day of November,2005,by and between the Board of County
Commissioners, on behalf of the Weld County Department of Social services,hereinafter referred to as"Social Services,"
and Compliance Administrative Services,hereinafter referred to as the"CAS".
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement(05-06-CPS-53)in order to acknowledge the
acquisition of Western Pathology Consultants,Inc./Mayfield by CAS and to extend the term of the original Agreement;
and
WHEREAS,Social Services has budgeted,appropriated,and otherwise made available; and
WHEREAS,required approval,clearance, and coordination have been accomplished from and with appropriate
agencies.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as follow:
1. The parties of the Agreement,as amended,will be Social Services and CAS. WCPI/Mayfield and its
holdings were acquired by CAS as of September 1,2005.
2. The designated representative of CAS shall be changed from Misty West,Chief Operations Manager to
Marc Pham,Controller.
3. Social Services agrees to increase the original amount for services from a$30,000 to$45,000 for
. services rendered.
4. Exhibit C,Collection Fee,will be revised as follows:
Instant 5 panel Drug Screen fee changed-$14.00
Standard Laboratory Test for ALC,AMP/METH,BENZ,COC, OPI,PCP THC-$13.00.
This Amendment,together with the original Child Protection Agreement for Services,constitutes the entire
understanding among the parties with respect to the subject matter hereof,and may not be changed or modified except as
stated in the Child Protection Agreement for Services.
IN WITNESS WHEREOF, the parties hereto have duly executed the Agreement as of the day,month, and year
first above written.
