RE: APPROVE TASK ORDER CHANGE ORDER LETTER #1 FOR BIOTERRORISM
PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with Task Order Change Order Letter#1 for the
Bioterrorism Program between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment,
commencing September 1, 2004, and ending August 31, 2005,with further terms and conditions
being as stated in said task order, and
WHEREAS,after review, the Board deems it advisable to approve said task order, a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Task Order Change Order Letter#1 for the Bioterrorism Program between
the County of Weld,State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Weld County Department of Public Health and Environment, and the
Colorado Department of Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said task order.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 13th day of July, A.D., 2005, nunc pro tunc September 1, 2004.

SUBJECT: Bioterrorism Task Order Change Order
Letter
Enclosed for Board review and approval is a task order change order letter for the Bioterrorism
Public Health Preparedness Program. If approved, this will become part of the Master Contract
between the Colorado Department of Public Health and Environment (CDPHE) and Weld
County on behalf of the Department of Public Health and Environment(WCDPHE).
With this change order letter, CDPHE is increasing funding of the original 2004-2005 task order
by $43,352 which brings the total amount available from the Bioterrorism grant to $330,883 for
the time period September 1, 2004 through August 31, 2005. This new funding will be used for
laboratory equipment and upgrades. Of the additional $43,352, approximately $8,962 is carry
forward funding from the 2003-2004 grant.
I recommend your approval of this task order change order letter.

This Task Order Change Order Letter is issued pursuant to paragraph F.3. of the Master Contract identified as
contract routing number 05 FAA 00054 and paragraph CA. of the Task Order identified as contract routing number
05 FHA 00106 and contract encumbrance number PO FHA EPI0500106. This Task Order Change Order Letter is
between the COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT and BOARD OF
COUNTY COMMISSIONERS OF WELD COUNTY (a political subdivision of the State of Colorado) for
the use and benefit of the Weld County Department of Public Health and Environment. The Task Order is
referred to as the "Original Task Order". This Task Order Change Order Letter is for the current term of
September 1. 2004 through August 31,2005. The maximum amount payable by the State for the work to be
performed by the Contractor during this current term is increased by Forty-Three Thousand Three Hundred
Fifty-Two Dollars,($43,352.00)for an amended total financial obligation of the State THREE HUNDRED
THIRTY THOUSAND, EIGHT HUNDRED THIRTY-THREE DOLLARS ($330,833.00). The revised
Budget is incorporated herein by this reference, made a part hereof, and attached hereto as"Attachment A". The
first sentence in paragraph C_1.of the Original Task Order is modified accordingly. All other terms and conditions
of the Original Task Order are reaffirmed. This change to the Task Order shall be effective upon approval by the
State controller, or designee, or on July 20,2005, whichever is later.
Please sign, date, and return all 04 originals of this Task Order Change Order Letter by July 10.2005, to the
attention of: Marti Wood,Contracts Coordinator, Colorado Department of Public Health and Environment,
4300 Cherry Creek Drive South, Mail Code DCEED 3650,Denver, Colorado 80246. One original of this Task
Order Change Order Letter will be returned to you when fully approved.
