RE: APPROVEAMENDMENTTO AGREEMENTFOR WELD COUNTY SHERIFF'S OFFICE
JAIL MEDICAL UNIT SERVICE AND AUTHORIZE CHAIR TO SIGN - DREAM TEAM
MEDICAL INCORPORATED
WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the EREAS,the Board has been presented with an Amendment to the Agreement for Weld
County Sheriffs Office Jail Medical Unit Service between the County of Weld,State of Colorado,by
and through the Board of County Commissioners of Weld County, on behalf of the Weld County
Sheriffs Office, and Dream Team Medical Incorporated, commencing upon full execution, with
further terms and conditions being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE,BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Amendment to the Agreement for Weld County Sheriffs Office Jail
Medical Unit Service between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County,on behalf of the Weld County Sheriffs Office,and Dream
Team Medical Incorporated be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was,on motion duly made and seconded, adopted by
the following vote on the 9th day of November, A.D., 2005.

In response to the 14% increase in the inmate population this 2005 contract price
adjustment is necessary. In order to meet the health care needs of the inmates at Weld
County Jail, increases in nursing staff, pharmaceuticals, supplies, support staff, and
provider clinic hours are needed. The specific monies are: for the additional nursing staff
$68,544.36, the pharmaceuticals and supplies $10,000, support staff$5,000, provider
clinics $6,000. In total the amount is $89,544.36. The County will pay the Provider the
additional funds in two equal payments of$44,772.18 due on or before the 10th of
November and December of 2005.
