RE: APPROVE AMENDMENT OF AGREEMENT TO PURCHASE CHILD PLACEMENT
AGENCY SERVICES AND AUTHORIZE CHAIR TO SIGN - EAGLE HOMES
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with an Amendment of Agreement to Purchase
Child Placement Agency Services between the County of Weld,State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of Social
Services, and Eagle Homes, commencing June 30, 2005, and ending September 30, 2005, with
further terms and conditions being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Amendment of Agreement to
Purchase Child Placement Agency Services between the County of Weld,State of Colorado, by and
through the Board of County Commissioners of Weld County,on behalf of the Department of Social
Services, and Eagle Homes be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 28th day of September, A.D., 2005, nunc pro tunc June 30, 2005.

RE: Amendments of Agreement to Purchase C' ' d Placement gency(CPA)
Services with Eagle Homes
Enclosed for Board approval are Amendment of Agreement to Purchase CPA Services between
the Weld County Department of Social Services (Department) and Eagle Homes. The
Amendment was reviewed at the Board's Work Session held on June 27,2005.
The Amendment provides for a 90 day extension to end September 3t, 2005, with all other
provisions in the original Agreement to remain the same.

THIS AMENDMENT made this day of 2005 between the Board of Weld County 1O
Commissioners,sitting as the Board of Social Services,on behalf of the Weld County Department of Social f..r6A herServices. "Contracer called tor"."County and Eagle Homes, 1225 Redwood St,Port Collins,CO 80524-2052, .9 called« 19).
WHEREAS,the Colorado State Department of Social Services,hereinafter called"State Department"is �e
authorized to provide social services to individuals and families of individuals through its agents,County t
Departments of Social Services,and
WHEREAS,County is authorized to purchase certain services for eligible children under State Department
rules,and
WHEREAS,certain meetings are occurring at the State level among the representatives from county
departments of social services,Colorado State Department of Social Services,and child placement agencies that
may affect assessment tools and related rates;and
WHEREAS,the stability of placements of children and youth under the guardianship of the County is vital;
and
WHEREAS,County and the Contracto wishes to amend its original agreement by extending the dunttion of
the Original Agreement entered into on�, ,2004;and
WHEREAS,Contractor is licensed as a Child Placement Agency.
NOW THEREFORE,it is hereby agreed that in consideration of the mutual Undertakings County and
Contractor agree as follows;
I. The term of the Agreement shall be extended from June 30,2005,to September 31,2005.
2. This Amendment,together with the original Agreement,constitutes the entire understanding among the parties
• with respect to the subject matter hereof,and may not be changed or modified except as stated in We original
Agreement.
