RE: APPROVE TASK ORDER RENEWAL LETTER#2 FOR COLORADO WOMEN'S
CANCER CONTROL INITIATIVE AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Task Order Renewal Letter#2 for the
Colorado Women's Cancer Control Initiative between the County of Weld, State of Colorado, by
and through the Board of County Commissioners of Weld County, on behalf of the Weld County
Department of Public Health and Environment, and the Colorado Department of Public Health
and Environment, commencing August 15, 2002, and ending September 30, 2002, with further
terms and conditions being as stated in said task order renewal letter, and
WHEREAS, after review, the Board deems it advisable to approve said task order
renewal letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Task Order Renewal Letter#2 for the Colorado Women's Cancer
Control Initiative between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment be,
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said task order renewal letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 31st day of July, A.D., 2002.

State Fiscal Year 2001-2002, Task Order Renewal Letter 2, Task Order Routing Number 03-00153
Pursuant to paragraph G of the Task Order with Task Order routing number 01-00030 and Task Order encumbrance number
PPG0100030 hereinafter referred to as the Original Task Order,as renewed with Task Order Renewal Letter 1,Task Order
routing number 02-00107,(copies of which are attached hereto and by this reference incorporated herein and made a part
hereof)between the State of Colorado,Department of Public Health and Environment and the Board of County
Commissioners of Weld County for the renewal term from August 15,2002 through September 30,2002 the parties
agree that the maximum amount payable by the State for the eligible services referenced in section B of the Original Task
Order is TWO THOUSAND,NINE HUNDRED AND EIGHTY-SEVEN DOLLARS($2,987.00)for a new total financial
obligation of the State of fifty-two thousand and seventy-five dollars ($52,075.00). The fast sentence in paragraph Cl of
the Original Task Order is hereby modified accordingly. All other terms and conditions of the Original Task Order are
hereby reaffirmed. This amendment to the Original Task Order is intended to be effective as of August 15,2002. However,
in no event shall this amendment be deemed valid until it shall have been approved by the State Controller or such assistant
as he may designate.
Please sign, date, and return all four originals of this Task Order Renewal Letter by July 31,2002,to the attention of:

SUBJECT: Task Order Renewal Letter#2 for the
CWCCI Program
Enclosed for Board review and approval is Renewal Letter Number Two to the task order
between Colorado Department of Public Health and Environment and Weld County Department
of Public Health and Environment for the outreach program of Colorado Women's Cancer
Control Initiative.
This letter is being issued to extend the current contract period until September 30, 2002 when a
new agreement will be issued according to Centers for Disease Control (CDC) guidelines.
Funding provided will be in the amount of$2,987 for WCDPHE to continue public education
and outreach activities to recruit women age 40 years and older for breast and cervical cancer
screening services during the time period August 15. 2002 through September 30, 2002.
I recommend your approval of this renewal letter.
